Title: The Effect of Anti-Diabetic Drugs on the Metabolism of Papillary Thyroid Cancer

Abstract: Background: Thyroid cancer (TC) is the most prevalent endocrine cancer and the 9th most prevalent cancer worldwide. The incidence rates have been increasing in the last decades, which has been attributed not only to overdiagnosis, but also to changes in environmental exposures and the concomitant increase in metabolic diseases, namely diabetes mellitus. The most common TC subtype, papillary thyroid cancer, responsible for 80-90% of new cases, presents an excellent prognosis in most cases. Nonetheless, 5-10% of patients develop aggressive metastatic disease, requiring further investigation on the molecular mechanisms involved. The underlying molecular pathology of PTC involves the dysfunction of signalling pathways, such as MAPK and PI3K/AKT/mTOR, related to cellular energy metabolism. Since these pathways are involved in metabolic disorders such as diabetes mellitus, this evidence paved the way for the study of anti-diabetic drugs, such as metformin, a mitochondrial complex I inhibitor, and canagliflozin, a sodium-glucose co-transporter 2 (SGLT2) inhibitor, as potential therapeutics for TC. Methods: The effect of anti-diabetic drugs metformin and canagliflozin on PTC cell lines was evaluated through transmission electronic microscopy (TEM) to assess changes in cell morphology, particularly mitochondrial changes. Glycolysis stress test was conducted to determine the ability of both drugs to induce metabolic shifts in the cells. Additionally, GLUT1, GLUT3, GLUT4, SGLT2, MCT1 and MCT4 protein expression was observed through immunofluorescence and quantified through western blot assay. Moreover, a proteomic analysis of the BCPAP, TPC-1 and Nthy Ori 3-1 cell lines was conducted. Results: TEM results show a significant canagliflozin-induced increase in mitochondrial total area in Nthy Ori 3-1 cell line, while metformin induced a significant increase in the number of lipid droplets in the TPC-1 cell line. Glycolysis stress test demonstrated the ability of canagliflozin to significantly empty the glycolytic reserve of BCPAP cells, whilst metformin induced significant decreases in the glycolytic reserve as a percentage in the BCPAP and Nthy Ori 3-1 cell lines. Glycolysis during drug response was also significantly lower after metformin treatment in both tumour cell lines. All transporters and co-transporters in study were observed through immunofluorescence. However, GLUT4 and SGLT2 were not observed through western blot analysis. Relatively to Nthy Ori 3-1 expression levels, BCPAP shows lower expression levels of GLUT1, GLUT3, MCT1 and MCT4, while TPC-1 only shows lower expression levels of the monocarboxylate family proteins. Proteomics analysis successfully identified differently expressed proteins and enriched Gene Ontology and Reactome pathways between the three cell lines and between both cancer cell lines versus the Nthy Ori 3-1 cell line. Conclusions: Both metformin and canagliflozin induced a metabolic shift in favour of glycolysis in papillary thyroid cancer cell lines. The results of this study indicate a potential role of anti-diabetic drugs as co-adjuvant therapy for papillary thyroid cancer.
Description: Introdução: O cancro da tiroide é o cancro mais frequente do sistema endócrino e o 9º mais prevalente a nível mundial. As taxas de incidência têm vindo a aumentar nas últimas décadas, o que é atribuído não só ao sobrediagnóstico, mas também a alterações nas exposições ambientais e ao aumento simultâneo das doenças metabólicas, nomeadamente a diabetes mellitus. O subtipo mais comum de cancro da tiroide, o carcinoma papilar da tiroide, responsável por 80-90% dos novos casos, apresenta na maioria das situações um excelente prognóstico. No entanto, 5-10% dos doentes desenvolvem doença metastática agressiva, o que requer investigação adicional sobre os mecanismos moleculares envolvidos. A patologia molecular subjacente ao carcinoma papilar da tiroide passa pela disfunção de vias de sinalização como as MAPK e PI3K/AKT/mTOR, associadas ao metabolismo energético celular. Sendo estas vias implicadas em distúrbios metabólicos como a diabetes mellitus, esta evidência abriu caminho ao estudo de medicamentos antidiabéticos, tais como a metformina (um inibidor do complexo mitocondrial I) e a canagliflozina (um inibidor do co-transportador sódio-glicose tipo 2 - SGLT2), como potenciais terapêuticas para o cancro da tiroide. Métodos: O efeito dos medicamentos antidiabéticos metformina e canagliflozina em linhas celulares de carcinoma papilar da tiroide foi avaliado por microscopia eletrónica de transmissão, de modo a observar alterações morfológicas, particularmente mitocondriais. Foi também realizado um teste de stress glicolítico para aferir a capacidade de ambos os medicamentos induzirem alterações metabólicas nas células. A expressão das proteínas GLUT1, GLUT3, GLUT4, SGLT2, MCT1 e MCT4 foi observada por imunofluorescência e quantificada por western blot. Adicionalmente, procedeu-se à análise proteómica das linhas celulares BCPAP, TPC-1 e Nthy Ori 3-1. Resultados: Os resultados de microscopia eletrónica de transmissão demonstram que a canagliflozina induziu um aumento significativo na área total mitocondrial na linha celular Nthy Ori 3-1, enquanto a metformina induziu um aumento significativo no número de gotas lipídicas na linha TPC-1. O teste de stress glicolítico demonstrou a capacidade da canagliflozina esgotar significativamente a reserva glicolítica nas células BCPAP, enquanto a metformina provocou diminuições significativas da reserva glicolítica como uma percentagem nas linhas BCPAP e Nthy Ori 3-1. Durante o tratamento, a glicólise também foi significativamente menor após a administração de metformina nas linhas celulares tumorais. Todos os transportadores e co-transportadores em estudo foram observados por imunofluorescência; porém, o GLUT4 e SGLT2 não foram detetados por western blot. Relativamente aos níveis de expressão da linha Nthy Ori 3-1, a BCPAP apresenta níveis inferiores de GLUT1, GLUT3, MCT1 e MCT4, enquanto a TPC-1 exibe apenas níveis mais baixos das proteínas da família dos transportadores de monocarboxilatos. A análise proteómica permitiu identificar proteínas diferencialmente expressas e vias enriquecidas do Gene Ontology e Reactome entre as três linhas celulares e entre as duas linhas tumorais e a Nthy Ori 3-1. Conclusão: Tanto a metformina com a Canagliflozina induziram uma alteração metabólica favorecendo a glicólise em linhas celulares de carcinoma papilar da tiroide. Os resultados deste estudo indiciam um potencial papel dos medicamentos antidiabéticos como terapia coadjuvante para o carcinoma papilar da tiroide.
