Title: Effect of baby-carrying and swaddling practices on developmental hip dysplasia: a systematic review

Abstract: Objective: To assess the association between different baby-carrying methods and swaddling and the occurrence of developmental dysplasia of the hip (DDH) in children under five years of age, through a systematic review. Study design: Medline (using PubMed), Scopus, and Web of Science were searched from inception to February 9, 2025. Observational studies that included samples of children ≤5 years old and reported the effect of swaddling and/or other baby-carrying methods on DDH were eligible. Two researchers independently assessed the inclusion criteria and evaluated risk of bias. Due to the heterogeneity of study designs and sample characteristics, a narrative synthesis was performed. Results: A total of 16 studies covering 50,377 children were included in this systematic review, ranging from 0 to 24 months of age. 4 of the studies were cross-sectional, 8 cohorts, and 4 were case-control. All reports evaluated swaddling as exposure, and most of the studies defined DDH through the Graf classification. A positive association between swaddling and the occurrence of DDH was observed in most of the included studies. Bilateral and late diagnosis of DDH was also more frequent among swaddled children. However, a sound assessment of the effect of baby-carrying methods and DDH was hampered by the limited number of studies, risk of bias, and lack of control for potential confounders. Conclusion: The present review highlights the relation between swaddling and DDH, and, therefore, this baby-carrying method should be used with caution. Description: Objetivo: Avaliar a associação entre os diferentes métodos de transporte de bebés e o swaddling com a ocorrência de displasia do desenvolvimento da anca (DDA) em crianças com idade inferior aos cinco anos, através de uma revisão sistemática. Desenho do estudo: A Medline (via a PubMed), a Scopus e a Web of Science foram pesquisadas até ao dia 9 de fevereiro de 2025. Foram elegíveis estudos observacionais que incluíram amostras de crianças ≤5 anos de idade e que reportaram sobre o efeito do swaddling e/ou outros métodos de transporte de bebés na DDA. Dois investigadores avaliaram independentemente os critérios de inclusão e o risco de viés. Devido à heterogeneidade dos desenhos de estudo e das caraterísticas da amostra, foi realizada uma síntese narrativa. Resultados: Esta revisão sistemática incluiu um total de 16 estudos que abrangeram 50 377 crianças, com idades compreendidas entre os 0 e os 24 meses de idade. 4 dos estudos eram transversais, 8 eram coortes e 4 eram casos-controlo. Todas as investigações realizadas avaliaram a exposição ao swaddling, e a maioria dos estudos diagnosticou a DDA através da classificação de Graf. Na maioria dos estudos incluídos, observou-se uma associação positiva entre o uso de swaddling e a ocorrência de DDA. O diagnóstico de doença bilateral e a apresentação tardia de DDA também foram mais frequentes entre as crianças em que foi utilizado o swaddling. No entanto, a avaliação do efeito dos métodos de transporte de bebés e da DDA foi dificultada pelo número limitado de estudos, pelo risco de viés e falta de controlo de potenciais confundidores. Conclusão: A presente revisão destaca a relação entre o swaddling e a DDA, e, por conseguinte, este método deve ser utilizado com cautela. Contudo, serão necessários mais estudos de modo a confirmar esta possível associação.
