| Title: | SGLT2i and aortic valve stenosis: impact on valve calcification |

| Abstract: | Background: Aortic valve stenosis (AVS) is a cardiovascular disease characterized by a continuous process of fibrosis and calcification mainly led by the activation of valvular interstitial cells (VICs). Despite its high prevalence, there is still no effective pharmacological therapies to prevent or to treat this disease. Sodium-glucose co-transporter 2 inhibitors (SGLT2i), such as empagliflozin (EMPA), have shown cardiovascular benefits, but their direct effects on VICs remain unclear. This study aimed to investigate the effects of SGLT2i on VICs'-mediated calcification. Methods: Human VICs were isolated from aortic valves of six patients (three men and three women) undergoing surgical valve replacement for severe AVS. Cells were isolated, expanded and cultured under either baseline conditions (VIC-GM) or pro-osteogenic medium (OM), with increasing concentrations of EMPA. After 21 days, cell viability was assessed using a Resazurin Assay, and calcification was quantified by Alizarin Red S staining, followed by image analysis. Results: VICs maintained viability above 70% at all EMPA concentrations tested, up to 100 µM, indicating good drug tolerance. However, significant reductions in viability were detected from 10 µM onward in men. Male-derived VICs exhibited significantly higher baseline calcification compared to female-derived VICs (66.4% vs. 38.0%, p=0.0003), suggesting intrinsic sex-related differences in osteogenic potential. Under osteogenic stimulation, EMPA reduced calcification in male VICs at 10 µM (11.6%, p=0.0392). In contrast, female VICs showed no significant reduction in calcification. These findings suggest a sex-specific cellular response to EMPA. Conclusions: EMPA may attenuate VIC-mediated calcification in a sex-specific manner, reducing calcium deposition in male-derived VICs but not in those from females. These findings suggest that SGLT2i could represent a novel pharmacological strategy for men with AVS and reinforce the importance of considering sex differences in the development of novel therapies. |
| Description: | Introdução: A estenose valvular aórtica é uma doença cardiovascular caracterizada por um processo contínuo de fibrose e calcificação, resultante, principalmente, da ativação de células intersticiais valvulares (do inglês, VICs). Apesar da sua elevada prevalência, ainda não existem terapias farmacológicas eficazes para prevenir ou tratar esta doença. Os inibidores do co-transportador 2 de sódio-glicose (do inglês, SGLT2i), como a empagliflozina (EMPA), têm demonstrado benefícios cardiovasculares, mas os seus efeitos diretos nas VICs permanecem por esclarecer. Este estudo teve como objetivo investigar os efeitos dos SGLT2i na calcificação mediada pelas VICs. Métodos: VICs humanas foram isoladas de válvulas aórticas de seis doentes (três homens e três mulheres) submetidos a substituição cirúrgica da válvula devido a estenose valvular aórtica grave. As células foram isoladas, expandidas e cultivadas em condições basais (VIC-GM) ou em meio pró-osteogénico (OM), com concentrações crescentes de EMPA. Após 21 dias, a viabilidade celular foi determinada através de um ensaio com Resazurina, e a calcificação foi quantificada por coloração com Vermelho de Alizarina S e subsequente análise de imagem. Resultados: As VICs apresentaram viabilidades acima de 70% em todas as concentrações de EMPA testadas, até 100 µM, indicando uma boa tolerância a este fármaco. No entanto, detetaram-se reduções significativas na viabilidade a partir de 5 µM em VICs masculinas. As VICs isoladas de homens apresentaram níveis de calcificação basais significativamente mais elevados do que as VICs de mulheres (66,4% vs. 38,0%, p=0,0003), sugerindo diferenças no potencial osteogénico intrínsecas ao sexo. Sob estimulação osteogénica, a EMPA reduziu a calcificação nas VICs masculinas a 10 µM (11.6%, p=0.0392). Em contrapartida, as VICs femininas não mostraram redução, e, paradoxalmente, apresentaram um aumento da calcificação com as concentrações de 5 e 10 µM (p=0,0366 e p=0,0283, respetivamente). Estes resultados sugerem uma resposta celular à EMPA específica por sexo. Conclusões: A EMPA atenuou a calcificação mediada por VICs de uma forma específica por sexo, reduzindo a deposição de cálcio em VICs isoladas de homens, mas não de mulheres. Estes resultados sugerem que os SGLT2i poderão representar uma nova estratégia farmacológica para os homens com estenose valvular aórtica e reforçam a importância de considerar as diferenças de sexo no desenvolvimento de novas terapias. |
