| dc.description | Contexto: O presente estudo tem como objetivo avaliar o volume da doença ganglionar axilar residual após quimioterapia neoadjuvante (QNA) no cancro da mama. Métodos: Foram analisados fatores preditores para doença ganglionar axilar residual de baixo volume em doentes com cancro da mama histologicamente confirmado (cT1-3 cN0-2), tratados com QNA e cirurgia entre 2015 e 2019. Resultados: Foram analisados 734 doentes. A resposta patológica completa (RPC) na mama foi observada em 35,1%, com 31,5% dos casos sem evidência de tumor residual na mama ou nos gânglios linfáticos (GLs) axilares. A taxa de RPC mamária e axilar variou de acordo com o subtipo molecular, sendo mais elevada nos tumores HER2+ e com recetores hormonais (RH) negativos. No subgrupo de 385 doentes com metástases axilares ao diagnóstico, 41,0% apresentaram RPC axilar e 38,4% revelaram 1 a 3 GLs metastizados. Os fatores preditores para RPC axilar incluíram o estádio cN, o estado dos RH, o estado ycN e a RPC mamária. Nos doentes cN+ ao diagnóstico que apresentaram RPC mamária, a taxa de RPC axilar foi de 81,1%. Após a QNA, 68,7% dos doentes que inicialmente apresentavam mais de 2 GLs metastizados apresentaram 0 a 3 GLs metastizados. Conclusão: A RPC mamária, o subtipo HER2+, o cancro da mama RH negativo, a presença de menos de 3 gânglios linfáticos axilares metastáticos ao diagnóstico e uma resposta imagiológica ganglionar axilar completa após a QNA estiveram associados a RPC axilar e a uma baixa probabilidade de mais de 3 gânglios linfáticos metastizados. Além disso, este estudo revelou que a RPC axilar em doentes com mais de dois gânglios metastizados ao diagnóstico foi semelhante à observada naqueles com 1-2 gânglios metastizados, destacando a possibilidade de personalizar a abordagem cirúrgica axilar neste subgrupo. | |
| dc.description.abstract | Background: The present study aims to evaluate the volume of residual axillary nodal disease after neoadjuvant chemotherapy (NACT) in breast cancer. Methods: Predictive factors for low-volume residual axillary nodal disease were analyzed in patients with histologically proven breast cancer (cT1-3 cN0-2) who were treated with NACT and surgery between 2015 and 2019. Results: A total of 734 patients were analysed. Breast pathological complete response (pCR) was achieved in 35.1%, with 31.5% showing no residual tumour in the breast or axillary lymph nodes (LNs). Breast and axillary pCR rates varied according to receptor subtype, with the highest rates found in HER2+ and hormone receptor (HR) negative tumours. Among 385 patients presenting with axillary metastases at diagnosis, 41.0% achieved axillary pCR, and 38.4% had 1-3 metastatic LNs. Predictive factors for axillary pCR included cN stage, HR status, ycN status, and breast pCR. For breast pCR patients with cN+ at presentation, the rate of axillary pCR was 81.1%. After NACT, 68.7% of patients with >2 metastatic LNs at diagnosis had 0-3 residual LNs. Conclusion: We conclude that breast pCR, HER2+, HR negative breast cancer, <3 metastatic axillary LNs at diagnosis, and complete axillary imaging response after NACT were associated with axillary pCR rates and low probability of >3 positive LNs. Similarly, our study showed that axillary pCR in patients with more than two metastatic LNs was comparable to that of patients with 1-2 metastatic LNs highlighting an opportunity to tailor axillary surgery in this subgroup. | |
