Title: Radiomics and Pericarotid Adipose Tissue Characteristics as Predictive Markers of Systemic Inflammation in Carotid Endarterectomy: A Cohort Study

Abstract: Background: Pericarotid adipose tissue (PCAT) characteristics and systemic inflammation play crucial roles in carotid endarterectomy (CEA) outcomes. This study investigates the relationship between PCAT Hounsfield Unit (HU) ranges, radiomics features, and inflammatory markers in CEA patients. Methods: Twenty patients undergoing CEA were included in this prospective study. PCAT was analyzed using computed tomography angiography (CTA) images, with regions of interest defined around the carotid arteries. PCAT was categorized into three HU ranges: -190 to -120, -119 to -70, and -69 to -30. Radiomics features were extracted using PyRadiomics. The primary outcome was the neutrophil-to-lymphocyte ratio (NLR), with red cell distribution width (RDW-CV) as a secondary outcome. Linear regression analysis was performed to assess associations between PCAT characteristics and inflammatory markers. Results: Different HU ranges in PCAT imaging showed strong correlations with NLR: the -190 to -120 range exhibited a negative association (β = -3.809, P < 0.001), while the -119 to -70 range showed a positive correlation (β = 3.814, P < 0.001). PCAT uniformity was positively associated with RDW-CV (β = 0.494, P = 0.027). Other radiomics features, such as contrast, showed trends but did not reach statistical significance. Larger outer areas of PCAT were inversely associated with NLR (β = -0.677, P < 0.001). Conclusion: Specific PCAT HU ranges and radiomics features are significantly associated with systemic inflammatory markers in CEA patients. That suggest that PCAT HU ranges and radiomics analysis, may provide valuable insights into the relationship between local adipose tissue characteristics and systemic inflammation.
Description: Introdução: As características do tecido adiposo pericarotídeo (PCAT) e a inflamação sistémica desempenham papéis cruciais nos desfechos da endarterectomia carotídea (CEA). Este estudo investiga a relação entre os intervalos de unidades Hounsfield (UH) do PCAT, as características radiómicas e os marcadores inflamatórios em doentes submetidos a CEA. Métodos: Foram incluídos neste estudo prospetivo 20 doentes submetidos a CEA. O PCAT foi analisado utilizando imagens de angiografia por tomografia computorizada (Angio-TC), com definição de regiões de interesse em torno das artérias carótidas. O PCAT foi categorizado em três intervalos de UH: -190 a -120, -119 a -70 e -69 a -30. As características radiómicas foram extraídas através do software PyRadiomics. O desfecho primário foi a relação neutrófilos-linfócitos (NLR), e o desfecho secundário foi a amplitude de distribuição dos glóbulos vermelhos (RDW-CV). A análise de regressão linear foi utilizada para avaliar associações entre as características do PCAT e os marcadores inflamatórios. Resultados: Os diferentes intervalos de UH do PCAT mostraram fortes correlações com a NLR: o intervalo -190 a -120 apresentou uma associação negativa (β = -3,809, P < 0,001), enquanto o intervalo -119 a -70 demonstrou uma correlação positiva (β = 3,814, P < 0,001). A uniformidade do PCAT esteve positivamente associada ao RDW-CV (β = 0,494, P = 0,027). Outras características radiómicas, como o contraste, apresentaram tendências, mas sem significância estatística. Áreas externas maiores de PCAT estiveram inversamente associadas à NLR (β = -0,677, P < 0,001). Conclusão:Intervalos específicos de UH do PCAT e características radiómicas estão significativamente associados a marcadores inflamatórios sistémicos em doentes submetidos a CEA. Estes achados sugerem que a análise dos intervalos de UH do PCAT e das características radiómicas pode fornecer informações valiosas sobre a relação entre as características do tecido adiposo local e a inflamação sistémica.
