| Title: | Overweight and obesity in type 1 diabetes: associations with glycemic control and cardiometabolic risk |

| Abstract: | Aims: This cross-sectional study aimed to investigate associations of body mass index (BMI) and waist circumference (WC) with glycemic control and other cardiometabolic risk factors in patients with type 1 diabetes (T1D). Methods: Patients followed in the Insulin Pump Clinic at a tertiary hospital were selected. Data from clinical records and continuous glucose monitoring (CGM)-derived parameters were retrospectively documented (2022-2024). Associations of BMI and WC with cardiometabolic risk factors were assessed using linear regression models. Restricted cubic splines analyses were performed to assess for nonlinear associations. Results: Of the 388 patients included, 32.3% were overweight and 13.7% obese. Patients with obesity and increased WC had higher systolic (p=0.013 and p=0.038) and diastolic blood pressure (p=0.001 and p=0.015), non-HDL cholesterol (p=0.022 and p=0.010) and triglycerides (p=0.001 and p<0.001) values. A non-linear association between BMI and glycated hemoglobin (HbA1c) was found, with higher HbA1c levels corresponding to the extreme BMI values. A higher time in range (p=0.036) was found in the increased WC group. Conclusions: In patients with T1D, BMI and WC were more strongly associated with blood pressure and lipid profile than with glycemic control. A better understanding of the role of obesity in the cardiometabolic risk of patients with T1D is essential to improve the management of this common comorbidity. | Description: | Objetivos: Este estudo transversal teve como objetivo investigar as associações do índice de massa corporal (IMC) e do perímetro da cintura (PC) com o controlo glicémico e outros fatores de risco cardiometabólico em pacientes com diabetes mellitus tipo 1 (DM1). Métodos: Foram selecionados doentes acompanhados na consulta de bomba infusora de insulina de um hospital terciário. Os dados dos registos clínicos e os parâmetros derivados da monitorização contínua da glicose (MCG) foram documentados retrospetivamente (2022-2024). As associações do IMC e do PC com fatores de risco cardiometabólico foram avaliadas através de modelos de regressão linear. Foram realizadas análises de splines cúbicos restritos para avaliar associações não lineares. Resultados: Dos 388 doentes incluídos, 32.3% apresentavam excesso de peso e 13.7% obesidade. Pacientes com obesidade e PC aumentado apresentavam valores mais elevados de pressão arterial sistólica (p=0.013 e p=0.038) e diastólica (p=0.001 e p=0.015), colesterol não-HDL (p=0.022 e p=0.010) e triglicerídeos (p=0.001 e p<0.001). Verificou-se uma associação não linear entre o IMC e a hemoglobina glicada (HbA1c), com níveis mais elevados de HbA1c correspondendo aos valores extremos de IMC. No grupo com PC aumentado, observou-se um maior tempo no alvo (p=0.036). Conclusões: Em doentes com DM1, o IMC e o PC mostraram uma associação mais forte com a pressão arterial e o perfil lipídico do que com o controlo glicémico. É essencial compreender melhor o papel da obesidade no risco cardiometabólico de doentes com DM1, de modo a melhorar a gestão desta comorbilidade. |
