Title: Definition and Predictors of Weight Regain after Metabolic Bariatric Surgery: a Systematic Review and Meta-Analysis

Abstract: Background: Despite the benefits of metabolic and bariatric surgery (MBS), recurrent weight gain (WR) - a significant weight gain after initial postoperative weight loss - remains a challenge for some patients. However, its definition varies, making it difficult to determine its true prevalence and its predictors. This study aims to systematically define WR and to identify clinically significant predictors of WR to improve patient counselling.   Methods: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations, we managed a Systematic Review (SR) and meta-analysis by searching three databases (PubMed, Scopus, and Web of Science). We performed a random meta-analysis of odds ratios (OR) and mean differences of factors associated with WR. Heterogeneity was assessed using the I2 statistic.   Results: 43 studies were included in the qualitative analysis and 10 in the quantitative analysis. Although there is no consensus on the true definition, in this review, the most common was the regain of ≥10% of nadir weight (46.5%). Related to its factors, age, preoperative Body Mass Index, time since surgery, diabetes mellitus, dyslipidemia, hypertension and gender were not statistically significant associated with higher risk of WR. Physical activity was the only significant protective factor of WR (OR = 9.92; 95% CI = [2.32; 42.45]; p=0.17), (I² = 46%, p = 0.17).   Conclusion: The use of multiple metrics with varying thresholds for WR highlights the need for a standardized definition. In this meta-analysis, WR results from an interplay of modifiable factors, in which physical inactivity seems to be the only independent risk factor.   Key Points: -       Recurrent weight gain (WR) - a significant weight gain after initial postoperative weight loss - remains a challenge; -       Multiple WR's definitions makes it difficult to understand its prevalence and clinical impact; Regular physical activity seems to have a protective effect on WR after MBS.
Description: Contexto: Apesar dos benefícios da cirurgia metabólica e bariátrica (MBS), o reganho de peso recorrente (WR) - um aumento significativo de peso após a perda inicial no pós-operatório - continua a ser um desafio para alguns doentes. No entanto, a sua definição varia, dificultando a determinação da verdadeira prevalência e dos fatores preditivos. Este estudo tem como objetivo definir sistematicamente o WR e identificar preditores clinicamente significativos para melhorar o aconselhamento dos doentes. Métodos: De acordo com as recomendações do PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses), realizámos uma Revisão Sistemática (RS) e uma meta-análise, pesquisando em três bases de dados (PubMed, Scopus e Web of Science). Conduzimos uma meta-análise random das odds ratios (OR) e das diferenças médias dos fatores associados ao WR. A heterogeneidade foi avaliada utilizando a estatística I². Resultados: Foram incluídos 43 estudos na análise qualitativa e 10 na análise quantitativa. Embora não exista consenso sobre a definição exata de WR, nesta revisão a mais comum foi o reganho de ≥10% do peso nadir (46,5%). Entre os fatores analisados, a idade, o Índice de Massa Corporal (IMC) pré-operatório, o tempo decorrido desde a cirurgia, a diabetes mellitus, a dislipidemia, a hipertensão e o género não apresentaram associação estatisticamente significativa com um maior risco de WR. A atividade física foi o único fator de proteção significativo contra o WR (OR = 9,92; IC 95% = [2,32; 42,45]; p=0,17), (I² = 46%, p = 0,17). Conclusão: A utilização de múltiplas métricas com diferentes critérios para definir WR reforça a necessidade de uma definição padronizada. Nesta meta-análise, o WR resulta da interação de fatores modificáveis, sendo a inatividade física o único fator de risco independente identificado. Pontos-chave: - O reganho de peso recorrente (WR) continua a ser um desafio após a cirurgia bariátrica; - A existência de múltiplas definições de WR dificulta a compreensão da sua prevalência e impacto clínico; A prática regular de atividade física parece ter um efeito protetor contra o WR após a MBS.
