Title: Genetic Dilated Cardiomyopathy and Response to Contemporary Heart Failure Therapy

Abstract: Introduction and Objectives: Since 2021, four therapeutic pillars-angiotensin receptor neprilysin inhibitor (ARNI) or ACE inhibitors /angiotensin receptor antagonists (ACEi/ARA), beta-blocker (BB), aldosterone receptor antagonist (ARM), and SGLT2 inhibitor (SGLT2i)-have been recommended for the treatment of patients with heart failure (HF) with reduced left ventricular ejection fraction (LVEF). The guidelines do not differentiate among etiologies of left ventricular dysfunction, however, the effects of these therapies specifically on genetic dilated cardiomyopathies remain unclear. In this retrospective study, we aimed to investigate the clinical response to the four pillars of therapy in a cohort of patients with familial dilated cardiomyopathy (fDCM) Methods: Patients diagnosed with fDCM were selected from heart failure and cardiomyopathy consultations at a tertiary hospital. The diagnostic criteria included the identification of pathogenic genetic variants in both sporadic and familial cases, as well as familial cases even if a conclusive genetic test was either not available or negative for VP. The analysis focused on patients who underwent serial echocardiograms after 2018. Echocardiographic changes from baseline to the last examination were determined, and only LVEF variation ≥10 percentage points were considered significant. The last visit recorded ongoing HF therapies, including the initiation date and dosage. Results: A total of 56 patients were included, 50% male, with a median age of 53 [range:20-79]. Pathogenic variants were identified in 32,1 % of patients. Initial LVEF was 40,5% [range: 17- 55], 10,7% of patients had atrial fibrillation, and 3,6% left bundle branch block. Fifty patients performed cardiac magnetic resonance, with late gadolinium enhancement present in 55,4%. During a follow-up period of 2,5 years, 96% of patients were treated with BB, 85,7% with ACEI/ARA, 59% ARNI, 79 % ARMs, and 93% with SGLT2i. The median LVEF at the last echocardiogram was 39,6% [range: 21- 57]. Six patients had an improvement in LVEF ≥ 10 points, and of these, only 3 reached a final LVEF >50%. In seven patients there was a reduction in LVEF in the same proportion, with a final value of 32% [range: 22- 41]. Most fDCM patients did not obtain a significant variation of LVEF despite optimized HF therapy. Conclusions: This study demonstrates that while the therapy using the four pillars is beneficial for patients with HF and reduced LVEF, the response may be insufficient for those with hereditary dilated cardiomyopathies. This highlights the need for clinical trials in these specific set of DCM patients.
Description: Introdução e Objetivos: Desde 2021, quatro pilares terapêuticos-inibidor da neprilisina do receptor de angiotensina (ARNI) ou inibidores da ECA/antagonistas do receptor de angiotensina (IECAs/ARAs), betabloqueadores (BB), antagonistas do receptor de aldosterona (ARM) e inibidores de SGLT2 (SGLT2i)-têm sido recomendados para o tratamento de pacientes com insuficiência cardíaca (IC) com fração de ejeção do ventrículo esquerdo reduzida (FEVE). As "guidelines" não diferenciam entre as etiologias da disfunção ventricular esquerda, contudo, os efeitos dessas terapias especificamente em cardiomiopatias dilatadas genéticas permanecem incertos. Neste estudo retrospectivo, objetivamos investigar a resposta clínica aos quatro pilares da terapia em uma coorte de pacientes com cardiomiopatia dilatada familiar (fDCM). Métodos: Realizamos um estudo retrospectivo no qual pacientes diagnosticados com fDCM foram selecionados das consultas de insuficiência cardíaca e cardiomiopatias de um hospital terciário. Os critérios de diagnósticos incluíram a identificação de variantes genéticas patogénicas em casos esporádicos e familiares, bem como casos familiares mesmo quando um teste genético conclusivo não estava disponível ou foi negativo para VP. A análise focou-se em pacientes que realizaram ecocardiogramas seriados após 2018. Alterações ecocardiográficas desde o início até o último exame foram determinadas, e apenas variações da FEVE ≥10 pontos percentuais foram consideradas significativas. Registaram-se as terapias de IC em curso, incluindo a data de início e dosagem, aquando do último ecocardiograma. Resultados: Um total de 56 pacientes foi incluído, 50% do sexo masculino, com idade mediana de 53 anos [20-79]. Variantes patogénicas foram identificadas em 32,1% dos pacientes. A FEVE inicial foi de 40,5% [17-55], 10,7% dos pacientes apresentavam fibrilhação auricular e 3,6% bloqueio do ramo esquerdo. Cinquenta pacientes realizaram ressonância magnética cardíaca, com realce tardio com gadolínio presente em 55,4%. Durante um período de acompanhamento de 2,5 anos, 96% dos pacientes foram tratados com BB, 85,7% com IECAs/ARAs, 59% com ARNI, 79% com ARMs e 93% com SGLT2i. A FEVE mediana no último ecocardiograma foi de 39,6% [21-57]. Seis pacientes tiveram uma melhora na FEVE ≥ 10 pontos e, destes, apenas 3 atingiram uma FEVE final >50%. Em sete pacientes houve uma redução na FEVE na mesma proporção, com um valor final de 32% [22-41]. A maioria dos pacientes com fDCM não obteve uma variação significativa da FEVE apesar da terapia otimizada para IC. Conclusões: Este estudo demonstra que, embora a terapia utilizando os quatro pilares seja benéfica para pacientes com IC e FEVE reduzida, a resposta pode ser insuficiente para aqueles com cardiomiopatias dilatadas hereditárias. Isso destaca a necessidade de ensaios clínicos neste conjunto específico de pacientes com DCM.
