Title: Anatomical study of the human upper part of the sacrum and ventral rami of the first and second sacral spinal nerves: relevance for sacral fractures management

Abstract: Purpose: This study aims to provide a comprehensive morphological description of the upper part of the sacrum and the ventral rami of the first and second sacral spinal nerves, focusing on their spatial relations with key bony landmarks to improve clinical outcomes in sacral fractures and pelvic trauma management. Furthermore, we intended to evaluate the possible correlation between the studied parameters and the size of the pelvis. Methods: Several measurements related to the bony landmarks of the upper part of the sacrum and the ventral rami of the first and second sacral spinal nerves were taken in 11 carefully dissected embalmed adult human cadavers. Results: Measurements of the ventral rami of the first and second sacral spinal nerves showed median lengths of 3.55 cm and 4.0 cm, respectively, with median widths of 0.9 cm and 0.7 cm, respectively. For the sacrum and sacral foramina, the median length of the anterior border of the sacral ala was 3.6 cm, and the vertical distance from this border to the upper limit of the first pelvic sacral foramen was 1.6 cm. The height and width of the first pelvic sacral foramen were 1.3 cm and 1.2 cm, respectively, while the second pelvic sacral foramen had a height and width of 1.0 cm each. The distance between the first and second sacral foramina was 1.3 cm. The anterior body height of the first and second sacral vertebrae measured in the median plane were 2.9 cm and 2.1 cm, respectively. We found a positive correlation between some of the analysed parameters and the size of the pelvis. Conclusion: The anatomical data of this study are crucial for surgical planning, helping to define safe corridors for instrumentation and reduce the risk of nerve injury during fixation procedures, leading to more effective stabilization and better patient outcomes.
Description: Objetivo: O objetivo deste estudo é fornecer uma descrição morfológica abrangente da porção superior do sacro e dos ramos ventrais do primeiro e segundo nervos espinhais sacrais, caracterizando as suas relações espaciais com pontos de referência ósseos fundamentais para melhorar os resultados clínicos nas fraturas do sacro e na gestão do trauma pélvico. Para além disso, pretendemos avaliar a possível correlação entre os parâmetros estudados e as dimensões da pelve. Métodos: Foram efetuadas várias medições relacionadas com os pontos de referência ósseos da porção superior do sacro e com os ramos ventrais do primeiro e segundo nervos espinais sacrais em 11 cadáveres humanos adultos embalsamados e cuidadosamente dissecados. Resultados: As medições dos ramos ventrais do primeiro e segundo nervos espinais sacrais revelaram comprimentos medianos de 3,55 cm e 4,0 cm, respetivamente, com larguras medianas de 0,9 cm e 0,7 cm, respetivamente. Em relação ao sacro e aos buracos sagrados pélvicos, o comprimento mediano do bordo anterior da asa do sacro foi de 3,6 cm, e a distância vertical desse bordo até o limite superior do primeiro buraco sagrado pélvico foi de 1,6 cm. A altura e a largura do primeiro buraco sagrado pélvico eram de 1,3 cm e 1,2 cm, respetivamente, enquanto o segundo buraco sagrado pélvico tinha uma altura e uma largura de 1,0 cm cada. A distância entre o primeiro e o segundo buraco sagrado pélvico era de 1,3 cm. A altura anterior do corpo das primeira e segunda vértebras sagradas, medida no plano mediano, foi de 2,9 cm e 2,1 cm, respetivamente. Foi identificada uma correlação positiva entre alguns dos parâmetros analisados e as dimensões da pelve. Conclusão: Os dados anatómicos obtidos neste estudo são de grande importância para o planeamento cirúrgico, ajudando a definir corredores seguros para a instrumentação e a reduzir o risco de lesão nervosa durante os procedimentos cirúrgicos, permitindo uma estabilização mais eficaz e melhores resultados para os pacientes.
