| dc.description | Objetivo: A lesão do ligamento cruzado anterior (LCA) é uma lesão prevalente, tanto no mundo desportivo, quanto na população geral, tornando a sua reconstrução um procedimento cirúrgico amplamente realizado em todo o mundo. Este estudo tem como objetivo analisar as diferenças biomecânicas em joelhos submetidos a reconstrução com enxertos autólogos de tendão dos isquiotibiais em quatro feixes ou de osso-tendão rotuliano-osso durante o teste de salto lateral. Métodos: Este estudo caso-controlo incluiu 46 participantes, sendo 18 indivíduos saudáveis (grupo controlo) e 28 com rotura do LCA submetidos a reconstrução com tendão dos isquiotibiais (n = 15) ou tendão rotuliano (n = 13). O sistema Theia Markerless e o software Visual3D permitiram obter informações sobre a cinemática do joelho, incluindo tempo de contato com o solo, varo e valgo máximos, e flexão durante valgo e varo máximos. Adicionalmente, a Escala de Atividade de Tegner permitiu avaliar a variação de nível de atividade física após a cirurgia. A análise e comparação dos dados foram realizadas por meio de ANOVA e teste de Tukey. Resultados: Foram encontradas diferenças estatisticamente significativas no tempo de contato com o solo entre os grupos com enxertos autólogos e o grupo controlo (p = 0,025, g = -1,13; p = 0,014, g = -1,09). No entanto, não houve diferenças entre os enxertos de tendão dos isquiotibiais e do tendão rotuliano. Nenhuma das outras variáveis estudadas apresentou diferenças estatisticamente significativas, mas observou-se uma leve tendência a maior valgo nos joelhos reconstruídos com enxerto rotuliano e maior varo nos joelhos reconstruídos com tendão dos isquiotibiais. Conclusões: Neste estudo, demonstramos que não foram encontradas diferenças na análise do salto lateral entre estas técnicas 12 meses após a cirurgia. A ausência de diferenças entre os grupos de intervenção e entre estes e o grupo controlo pode sugerir que ambos os enxertos permitem a restauração da cinemática do joelho por meio de mecanismos neuromusculares e propriocetivos que compensam as alterações anatómicas. | |
| dc.description.abstract | Purpose: Anterior cruciate ligament injury is a prevalent lesion both in the athletic world as well as in the general population, rendering its' reconstruction a popular surgical procedure worldwide. This study aims to analyze the biomechanical differences in knees that underwent reconstruction with four-strand hamstring or bone-patellar tendon-bone autografts during side-hop test. Methods: This case-control study included 46 participants, 18 were healthy controls and 28 had anterior cruciate ligament tears and were subjected to reconstruction either with hamstring (n = 15) or patellar (n = 13) tendons. Theia Markerless system and Visual3D allowed us to obtain information on knee kinematics, namely time of contact to the ground, maximum varus and valgus and flexion during maximum valgus and varus. Additionally, Tegner Activity Scale enabled the evaluation of activity level variation post-surgery. The data analysis and comparison were performed by using ANOVA and Tukey. Results: Differences were found in terms of contact time with the ground between the autograft groups and the control group (p = 0.025, g = -1.13; p = 0.014, g = -1.09). However, no differences were shown between hamstring and patellar tendon autografts. None of the other variables studied showed statistically significant differences, however it was seen a slight tendency to greater valgus in autografts with patellar autografts and bigger varus on the knees reconstructed with hamstring tendons. Conclusions: In this study we demonstrated that no differences on side hop analysis were found between these techniques 12 months after surgery. The lack of differences between the intervention groups, and between these and the control group, may suggest that both autografts allow knee kinematics restoration resorting to neuromuscular and proprioceptive mechanisms that overcome the anatomical alterations. | |
