Title: Association of thyroid function within the normal range with glycemic control and cardiovascular risk in type 1 diabetes

Abstract: Purpose: Changes in thyroid hormone (TH) levels, even within the normal range, influence cardiometabolic health. In type 1 diabetes (T1D), optimal control of glucose levels and cardiovascular risk is crucial to prevent complications. We aim to explore the association of TH within normal range with cardiometabolic profile in T1D. Methods: Cross-sectional analysis including adult patients with T1D followed at our Endocrinology Department between 2022-2024. We excluded patients with TSH or fT4 outside the reference range. We analyzed the associations between TH (TSH, fT4, fT3 and fT3/fT4 ratio) and glycemic, anthropometric, and cardiometabolic parameters, using linear regression models unadjusted and adjusted for relevant variables. Restricted cubic splines between TH and glycemic control parameters were performed to assess non-linear associations. Results: We included 296 patients (median age 35 [25.0, 47.0] years, 42.6% female). Those with TSH in the middle tertile had a higher time in range and a lower time above range. Waist circumference (WC) and body mass index (BMI) were negatively associated with fT4, and positively with fT3 and Ft3/fT4 ratio. Estimated glomerular filtration rate (eGFR) was negatively associated with TSH and fT4, and positively with fT3 and fT3/fT4 ratio. Conclusions: Variations in TH within the normal range were associated with glycemic control and cardiovascular risk in T1D. Patients with low-normal and high-normal TSH had a poorer glycemic control. Patients with lower fT4 and higher fT3 and fT3/fT4 ratio had higher BMI and WC. Finally, those with higher fT4 and lower fT3 and fT3/fT4 ratio presented a worse kidney function.
Description: Introdução: Variações nos níveis das hormonas tiroideias (HT), mesmo dentro do intervalo de referência, podem influenciar a saúde cardiometabólica. Na diabetes tipo 1 (DM1), a otimização do controlo glicémico e do risco cardiovascular é crucial para prevenir complicações. Este estudo tem como objetivo explorar a associação das HT, dentro da faixa da normalidade, com o perfil cardiometabólico em doentes com DM1. Métodos: Realizamos uma análise transversal incluindo doentes adultos com DM1 seguidos no nosso Serviço de Endocrinologia entre 2022 e 2024. Excluímos doentes com valores de TSH ou T4 livre (T4L) fora do intervalo de referência. Analisámos as associações entre as HT (TSH, T4L, T3 livre [T3L] e o rácio T3L/T4L) e parâmetros glicémicos, antropométricos e cardiometabólicos, utilizando modelos de regressão linear não ajustados e ajustados para variáveis clinicamente relevantes. Foram também realizadas análises com splines cúbicos restritos para avaliar associações não lineares entre as HT e os parâmetros de controlo glicémico. Resultados: Incluímos 296 doentes (idade mediana de 35 [25,0; 47,0] anos, 42,6% do feminino). Os doentes com TSH no tercil médio apresentaram maior tempo no alvo e menor tempo acima do alvo. O perímetro abdominal (PA) e o índice de massa corporal (IMC) revelaram uma associação negativa com a T4L e positiva com a T3L e o rácio T3L/T4L. A taxa de filtração glomerular estimada (TFGe) associou-se negativamente com a TSH e com a T4L, e positivamente com a T3L e o rácio T3L/T4L. Conclusões: Variações nas HT dentro do intervalo de referência associaram-se de forma significativa com o controlo glicémico e risco cardiovascular em doentes com DM1. Doentes com TSH nos limites inferior e superior da normalidade apresentaram pior controlo glicémico. Aqueles com T4L mais baixa e T3L e rácio T3L/T4L mais elevados, a sugerir um aumento da conversão da T4L em T3L, apresentaram maior IMC e PA. Por fim, doentes com T4L mais elevada, e T3L e rácio T3L/T4L mais baixos revelaram ter pior função renal.
