Title: Validation and acceptability of the mobile app version of the Control of Allergic Rhinitis and Asthma Test for Children (CARATKids): a cross-sectional study

Abstract: Background: The electronic version of the Control of Allergic Rhinitis and Asthma Test for Children (CARATkids) has the potential to enhance pediatric telemonitoring between clinical appointments but is not yet validated. Objective: This study aimed to validate the electronic version of CARATKids against the gold standard paper-based version. Methods: A cross-sectional study was conducted between April and December 2024 in a tertiary hospital in northern Portugal. Children with asthma and/or allergic rhinitis and their caregivers were recruited during pulmonology outpatient appointments. CARATKids comprises 13 yes/no questions, 8 addressed to the child and 5 to the caregiver, and the total score ranges from 0 to 13. The electronic CARATKids was implemented in a mobile app. Both paper and electronic versions were administered in a randomized order before and after the appointment. Additionally, participants' preferences between the two administration versions were assessed. Internal consistency (Cronbach's alpha), reliability (intraclass correlation coefficient-ICC, Bland-Altman analysis) and convergent validity (Spearman's coefficient) were analyzed following COnsensus-based Standards for selection of health status Measurement Instruments (COSMIN) guidelines. Results: Fifty-one children (median 9 [Q1-Q3 8-11] years; 56.9% male) and respective caregivers were included. The median CARATKids total score was similar across the paper (5 [3-8]) and electronic (5 [3-7]) versions. The internal consistency was 0.79 for the paper version and 0.83 for the electronic version. The reliability between the two versions was excellent (ICC 0.95, 95% CI: 0.91-0.97). The Bland-Altman analysis showed strong agreement between the two versions, with a mean difference of 0.04 (95% limits of agreement: -1.99-2.07). The Spearman's correlation between the two versions was 0.95 (p<0.001). In total, 62.7% of children and 60.8% of caregivers were indifferent to the version used, while 33.3% and 35.3%, respectively, preferred the electronic format. Conclusions: The electronic version of CARATKids appears to be equivalent to the paper-based version of the questionnaire, with good acceptance by children and caregivers. Its implementation in mHealth technologies has the potential to enhance remote child monitoring and optimize the management of asthma and allergic rhinitis.
Description: Enquadramento: A versão eletrónica do Teste de Controlo da Asma e Rinite Alérgica para Crianças (CARATKids) tem o potencial de melhorar a telemonitorização pediátrica entre consultas de seguimento. No entanto, esta versão ainda não foi validada. Objetivo: Este estudo teve como objetivo validar a versão eletrónica do CARATKids em comparação com a versão tradicional em papel. Métodos: Foi realizado um estudo transversal entre abril e dezembro de 2024 num hospital terciário, no norte de Portugal. Crianças com asma e/ou rinite alérgica entre os 6-12 anos e os seus cuidadores foram recrutados durante consultas externas de pneumologia. O CARATKids é composto por 13 perguntas de resposta sim/não, 8 direcionadas à criança e 5 aos cuidadores, e a sua pontuação total varia entre 0 e 13. A versão eletrónica do CARATKids foi implementada numa aplicação móvel. As versões em papel e eletrónica foram administradas aleatoriamente antes e depois da consulta. Além disso, foram avaliadas as preferências dos participantes em relação à versão utilizada. A consistência interna (alfa de Cronbach), a fiabilidade (coeficiente de correlação intraclasse, análise de Bland-Altman) e a validade convergente (correlação de Spearman) foram analisadas de acordo com a iniciativa COnsensus-based Standards for the selection of health status Measurement Instruments (COSMIN). Resultados: Foram incluídas 51 crianças (mediana de 9 [Q1-Q3 8-11] anos; 56.9% do sexo masculino) e respetivos cuidadores. A pontuação mediana do CARATKids foi semelhante na versão em papel (5 [3-8]) e na versão eletrónica (5 [3-7]). A consistência interna foi de 0.79 para a versão em papel e 0.83 para a versão eletrónica. A fiabilidade entre as duas versões foi de 0.95 (IC 95%: 0.91-0.97). A análise de Bland-Altman demonstrou uma forte concordância entre as duas versões, com uma diferença média de 0.04 (limites de concordância de 95%: -1.99-2.07). A correlação de Spearman entre as duas versões foi de 0.95 (p<0.001). No total, 62.7% das crianças e 60.8% dos cuidadores foram indiferentes à versão utilizada, enquanto 33.3% e 35.3%, respetivamente, preferiram a versão eletrónica. Conclusão: A versão eletrónica do CARATKids parece ser equivalente à versão em papel do questionário, com boa aceitação por parte das crianças e dos cuidadores. A sua implementação em tecnologias de saúde móvel tem o potencial de melhorar a monitorização remota das crianças e otimizar a gestão da asma e rinite alérgica.
