| Title: | A Sobrecarga do Cuidador Informal da Pessoa com Doença Oncológica em Cuidados Paliativos em Portugal |

| Abstract: | Introduction: The phenomenon of caregiver burden significantly influences the care experience, both for caregivers and patients. Despite the recognized importance of informal caregivers in the care of cancer patients in palliative care, there are still few studies exploring the burden that these caregivers experience in the national context. Objectives: This study's main objectives were to determine the level and type of burden of informal caregivers of palliative adult cancer patients in Portugal, through the application of an instrument to assess caregiver burden, to analyze the relationship between the burden of informal caregivers and the variables related to the informal caregiver, the care process and the patient, explore risk or protective factors of burden and, finally, establish a comparative analysis of the study results with the existing literature. Methodology: To respond to the formulated objectives, a quantitative, cross-sectional study was designed to characterize the burden of informal caregivers of adult cancer patients undergoing palliative care at a Local Health Unit in the North of the country. The study included unpaid adult and primary informal caregivers who continuously accompanied and supported the patient, also an adult, in at least one ADL. Burden was analyzed using the Portuguese version of the Zarit caregiver burden scale. The relationship between caregiver burden and related variables was established using multiple linear regression analyses. The study followed the formal and ethical procedures inherent to scientific research, including formal consent from participants. Results: The burden predominantly found in the caregivers under this study was intense, with an emotional and economic nature. A final burden risk statistical model consisting of the variables age of the caregiver, financial situation of the caregiver, gender of the patient and use of chronic medication by the caregiver to control pain, made it possible to explain around 30% of the burden of the caregivers in the study. The significant findings of the present investigation were consistent with the existing literature. Conclusions: Caregiver burden is multidimensional, so the approach is complex. The present study contributed to greater knowledge, analysis and reflection on the problem involving the informal caregiver of adult cancer patients undergoing palliative care in the national context. It is suggested that health policies need to pay special attention to this population of caregivers, considering the associated risk factors. The development of new similar primary studies, with methodological adjustments, as well as longitudinal studies is encouraged. The inclusion of new variables, of a psychosocial nature, such as hope, coping styles and spiritual intelligence are possible emerging challenges in this topic. |
| Description: | Introdução: O fenómeno da sobrecarga do cuidador influencia de forma determinante a experiência de cuidado, quer para os cuidadores, quer para os doentes. Apesar da reconhecida importância do cuidador informal no cuidado ao doente oncológico em Cuidados Paliativos, são ainda escassos os estudos que exploram a sobrecarga que estes cuidadores experienciam no contexto nacional. Objetivo: Este estudo teve como objetivos principais determinar o nível e tipo de sobrecarga dos cuidadores informais de doentes oncológicos adultos paliativos em Portugal, por meio de aplicação de um instrumento de avaliação da sobrecarga do cuidador, analisar a relação entre a sobrecarga dos cuidadores informais e as variáveis relacionadas com o cuidador informal, o processo de cuidar e o doente, explorar fatores de risco ou protetores da sobrecarga e, por fim, estabelecer uma análise comparativa dos resultados do estudo com a literatura existente. Material e Métodos: Para dar resposta aos objetivos formulados desenhou-se um estudo quantitativo, transversal de caracterização da sobrecarga dos cuidadores informais de doentes oncológicos adultos em Cuidados Paliativos de uma Unidade Local de Saúde da região Norte do país. Incluíram-se no estudo cuidadores informais principais, adultos, não remunerados, que acompanhassem e apoiassem de forma continuada o doente, também ele adulto, em pelo menos uma AVD. A sobrecarga foi analisada com recurso à versão portuguesa da escala de sobrecarga do cuidador de Zarit. A relação entre a sobrecarga dos cuidadores e as variáveis relacionadas foi estabelecida recorrendo a análises de regressão linear múltipla. O estudo seguiu os procedimentos formais e éticos inerentes à investigação científica, incluindo o consentimento formal dos participantes. Resultados: Foram analisados 64 pares cuidador informal/doente. A sobrecarga predominantemente encontrada nos cuidadores em estudo foi a sobrecarga intensa, de natureza emocional e económica. Um modelo estatístico final de risco de sobrecarga constituído pelas variáveis idade do cuidador, situação financeira do cuidador, género do doente e uso por parte do cuidador de medicação crónica para controlo da dor, permitiu explicar cerca de 30% da sobrecarga dos cuidadores em estudo. Os achados significativos da presente investigação foram consistentes com a literatura existente. Conclusão: A sobrecarga do cuidador é multidimensional, portanto a abordagem é complexa. O presente estudo contribuiu para um maior conhecimento, análise e reflexão acerca da problemática que envolve o prestador de cuidados informal do doente oncológico adulto em Cuidados Paliativos no contexto nacional. Sugere-se a necessidade das políticas de saúde prestarem atenção especial a esta população de cuidadores, considerando os fatores de risco associados. É encorajado o desenvolvimento de novos estudos primários semelhantes, com ajustes metodológicos, bem como os estudos do tipo longitudinal. A inclusão de novas variáveis, de natureza psicossocial, nomeadamente a esperança, os estilos de coping e a inteligência espiritual são possíveis desafios emergentes neste tema. |
