Title: Understand the Role of Glycans in the Oncogenesis of Pediatric Sarcomas

Abstract: Sarcoma, a cancer originating from mesenchymal cells, is more prevalent in children and adolescents (<18) than in adults. The most common pediatric sarcoma (PS) subtypes are rhabdomyosarcoma (RMS), osteosarcomas (OS) and Ewing sarcoma (ES). Diagnosing and treating these cancers remains a clinical challenge, highlighting the need for new biomarkers to improve diagnosis and develop more effective therapies. Aberrant glycans expression is a feature in cancer, with alterations in cellular glycosylation having a role in tumor development and progression. This thesis aimed to characterize aberrant glycan expression in the most common PS subtypes and explore their biological roles in PS oncogenesis. We started by a retrospective epidemiological analysis of PS cases in the North of Portugal. Further, we assessed, by immunofluorescence and flow cytometry analysis, the expression of described cancer-associated glycans, in commercially available cell lines from the three most prevalent PS subtypes. To unveil the biological roles of the identified glycans, the cell lines were glycoengineered, by CRISPR/Cas9 technology, and parameters such as proliferation, motility, invasion and activation of receptor tyrosine kinase (RTK) were assessed. Promising identified glycans were further validated in tissues samples from our retrospective patient cohort. Our epidemiological study revealed that along the past 23 years, 15 different PS subtypes were diagnosed, with ES, RMS and OS being the most common. A differential expression of cancer-associated glycans was observed across the different cell lines in study. Terminal α2,6 sialylation was highly expressed in ES and RMS cell lines, while heparan sulfate (HS) was prominent in the OS cell line. To investigate their functional roles, we knocked-out (KO) the enzymes responsible for their biosynthesis, resulting in the loss of terminal α2,6 sialylation in ES and RMS KO clones and a substantial reduction of HS in OS KO clones. Additionally, KO of HS led to a marked decrease in metabolic activity of OS cells, as well as reduction in the activation of few RTK such as PDGFRα. Ultimately, HS expression was validated in tissues samples from pediatric OS patients from our cohort, with positivity observed in seven out of ten cases.
Description: Sarcomas, um tipo de cancro com origem em células mesenquimais, é mais prevalente em crianças e adolescentes (<18 anos) do que em adultos. Os subtipos mais comuns de sarcomas pediátricos (SP) são o rabdomiossarcoma (RMS), o osteossarcoma (OS) e o sarcoma de Ewing (ES). O diagnóstico e o tratamento destes sarcomas permanecem um desafio clínico, reforçando a necessidade de descoberta de novos biomarcadores clínicos para o diagnóstico e desenvolvimento de terapias mais eficazes. A expressão alterada de glicanos é uma característica em cancro, desempenhando um papel fulcral no desenvolvimento e progressão tumoral. Esta dissertação teve como objetivo caracterizar a expressão de glicanos aberrantes nos subtipos mais comuns de SP e analisar possíveis papeis biológicos na sua oncogénese. Desta forma, foi realizado um estudo epidemiológico retrospetivo de pacientes com SP no Norte de Portugal. A avaliação da expressão de glicanos associados ao cancro foi efetuada por analises imunofluorescência e citometria de fluxo em linhas celulares derivadas dos três subtipos mais prevalentes de SP. Para decifrar possíveis papéis biológicos dos glicanos identificados, as linhas celulares foram editadas geneticamente usando a tecnologia CRISPR/Cas9, e avaliados parâmetros como proliferação, motilidade, invasão e ativação de recetores tirosina quinase. A avaliação dos glicanos identificados foi também efetuada em amostras de tecidos da nossa coorte de pacientes. O nosso estudo epidemiológico revelou que nos últimos 23 anos foram diagnosticados casos correspondentes a 15 subtipos diferentes de SP, sendo ES, RMS e OS os mais comuns. Observamos uma expressão diferencial de glicanos associados ao cancro nas diferentes linhas celulares estudadas. A sialilação terminal do tipo α2,6 está presente nas linhas celulares de ES e RMS, e heparan sulfato (HS) predominante presente na linha celular de OS. O silenciamento das enzimas responsáveis pela biossíntese dos glicanos identificados resultou na perda quer de sialilação terminal do tipo α2,6 nas células de ES e RMS, quer na redução substancial de HS em células de OS. A redução de HS levou a uma diminuição acentuada na atividade metabólica das células de OS, assim como na redução da ativação de recetores de tirosina quinase, nomeadamente de PDGFRα. Por último, a expressão em tecidos de HS foi validada em sete de dez casos de pacientes pediátricos com OS.
