Title: Differential Expression of Metabolic Enzymes in Testicular Germ Cell Tumors: The Impact of Glycolytic Metabolism on the Embryonal Carcinoma Histotype

Abstract: INTRODUCTION: Testicular germ cell tumors (TGCTs) are the most common solid malignancies in young adult males, with increasing incidence globally. These tumors are broadly categorized into seminomas (SEs) and non-seminomas (NSs), each with distinct biological behaviors and treatment responses. Despite advances in diagnosis and treatment, the understanding of TGCTs at the molecular level remains limited, necessitating further research into the metabolic pathways and genetic alterations that drive these malignancies. The exploration of metabolic enzymes and their expression in TGCTs can offer new insights into tumor behavior, potentially unveiling novel therapeutic targets. OBJECTIVES/AIMS: This thesis aims to investigate the differential expression of GLUT-1, MCT1, MCT4, LDHA, HIF-1α, as well as the epigenetic enzyme KDM3A, across various histological subtypes of TGCTs, including SEs, embryonal carcinomas (ECs), teratomas (TEs), yolk sac tumors (YSTs), and mixed germ cell tumors (mixed GCTs). By analyzing these markers, the study seeks to elucidate the impact of glycolytic metabolism on the aggressiveness of specific histotypes, particularly focusing on the embryonal carcinoma subtype. Additionally, the research will incorporate clinicopathological data to correlate metabolic expression patterns with patient outcomes, providing a comprehensive understanding of the metabolic underpinnings of TGCTs and paving the way for more targeted therapeutic strategies. MATERIALS AND METHODS: This study analyzed a total of 111 tumor components from 90 patients with a median age of 30 years. The samples included a total of 74 GCNIS-related TGCTs, comprising 37 seminoma components (SEs), 29 embryonal carcinoma components (ECs), 21 postpubertal-type teratoma components (TEs) 20 yolk sac tumor components (YSTs) and 4 choriocarcinoma components. Additionally, 16 GCNIS-unrelated tumors were studied, including 10 prepubertal-type TEs, 4 prepubertal-type yolk sac tumors (YSTs), and 2 spermatocytic tumors. In silico analysis was performed in cBioPortal. Immunohistochemistry was performed to evaluate the expression of GLUT-1, MCT1, MCT4, LDHA, HIF-1α and KDM3A. The staining was assessed by an experienced pathologist, scoring both the percentage of positive cells and the intensity of staining. Statistical analysis included the Mann-Whitney U test and Kruskal-Wallis test for continuous variables, with ANOVA employed to assess differences in marker expression among histological subtypes and disease stages. Spearman's correlation coefficient was used to determine correlations between continuous variables. Results were considered statistically significant at p-values < 0.05. RESULTS: Our analysis revealed significant differences in the expression of key metabolic markers among various TGCT subtypes, confirming the in silico analysis performed. GLUT-1, MCT4, MCT1 and LDHA showed higher expression in non-seminomas (NSs) compared to seminomas (SEs). Embryonal carcinomas (ECs) displayed the highest expression levels of GLUT-1, MCT4, MCT1 and LDHA, significantly exceeding those in SEs, teratomas (TEs), and yolk sac tumors (YSTs). HIF-1α expression was generally low across all subtypes but was still higher in ECs compared to SEs. KDM3A expression was notably higher in SEs than in NSs and mixed GCTs. Additionally, no significant differences in KDM3A expression were observed across different disease stages. CONCLUSION: The differential expression patterns of metabolic markers underscore the distinct metabolic profiles of TGCT subtypes, particularly the heightened glycolytic activity in ECs, which is in line with the morphology of this tumor component, which is highly necrotic and proliferative. These findings highlight the potential of targeting glycolytic pathways for therapeutic interventions in TGCTs, especially for aggressive subtypes like ECs. The study provides a comprehensive molecular characterization that could inform future research and treatment strategies for TGCTs.
Description: INTRODUÇÃO: Os tumores de células germinativas do testículo (TGCTs) são as neoplasias sólidas malignas mais comuns em homens jovens, com incidência global crescente. Estes tumores são amplamente categorizados em seminomas (SEs) e não seminomas (NSs), cada um com comportamentos biológicos e respostas ao tratamento distintos. Apesar dos avanços no diagnóstico e tratamento, a compreensão dos TGCTs ao nível molecular ainda é limitada, exigindo, assim, novas pesquisas sobre as vias metabólicas e as alterações genéticas que impulsionam estas neoplasias. A investigação de enzimas metabólicas e sua expressão nos TGCTs pode oferecer novas perspetivas sobre o comportamento tumoral, potencialmente revelando alvos terapêuticos inovadores. OBJETIVOS: Esta tese tem como objetivo investigar a expressão diferencial de GLUT-1, MCT1, MCT4, LDHA, HIF-1α e KDM3A entre os vários subtipos histológicos de TGCTs, incluindo SEs, carcinomas embrionários (ECs), teratomas (TEs), tumores do saco vitelino (YSTs) e tumores de células germinativas mistos (GCTs mistos). Ao analisar esses marcadores, o estudo procura elucidar o impacto do metabolismo glicolítico na agressividade de subtipos específicos, com foco particular nos ECs. Adicionalmente, a pesquisa incorporará dados clinico-patológicos para correlacionar padrões de expressão metabólica com os resultados dos pacientes, proporcionando uma compreensão abrangente das bases metabólicas dos TGCTs e pavimentando o caminho para estratégias terapêuticas mais direcionadas. MATERIAIS E MÉTODOS: Este estudo analisou um total de 111 componentes, provenientes de 90 pacientes com idade média de 30 anos. As amostras incluíram um total de 74 TGCTs relacionados com GCNIS, compreendendo 37 componentes de SE, 29 componentes de EC, 21 componentes de teratomas de tipo pós-pubertário (TEs), 20 componentes de YSTs de tipo pós-pubertário e 4 componentes de coriocarcinoma (CH). Adicionalmente, foram estudados 16 tumores não relacionados com GCNIS, incluindo 10 TEs de tipo pré-pubertário, 4 YSTs de tipo pré-pubertário e 2 tumores espermatocíticos. A análise in silico foi realizada com recurso à plataforma cBioPortal. A avaliação da expressão de GLUT-1, MCT1, MCT4, LDHA, HIF-1α e KDM3A foi realizada através de imunohistoquímica. As lâminas foram avaliadas por um patologista experiente, que pontuou tanto a percentagem de células positivas quanto a intensidade da marcação. A análise estatística incluiu o teste de Mann-Whitney e o teste de Kruskal-Wallis para variáveis contínuas, com recurso ao teste ANOVA para avaliar diferenças na expressão dos marcadores entre subtipos histológicos e estadios da doença. O coeficiente de correlação de Spearman foi utilizado para determinar correlações entre variáveis contínuas. Os resultados foram considerados estatisticamente significativos com valores de p < 0,05. RESULTADOS: A nossa análise revelou diferenças significativas na expressão dos principais marcadores metabólicos entre os vários subtipos de TGCT, em linha com a análise in silico realizada. GLUT-1, MCT4, MCT1 e LDHA apresentaram maior expressão em NSs, em comparação com os SEs. Os ECs exibiram os níveis mais altos de expressão de GLUT-1, MCT4, MCT1 e LDHA, superando significativamente os dos SEs, TEs e YSTs. A expressão de HIF-1α foi globalmente baixa em todos os subtipos, mas ainda assim, mais elevada nos ECs em comparação com os SEs. A expressão de KDM3A foi notavelmente maior nos SEs do que nos NSs e GCTs mistos. Além disso, não foram observadas diferenças significativas na expressão de KDM3A em diferentes estadios da doença. CONCLUSÃO: Os padrões diferenciais de expressão dos marcadores metabólicos destacam os perfis metabólicos distintos dos subtipos de TGCT, particularmente a atividade glicolítica aumentada nos ECs, o que se coaduna com a morfologia deste subtipo histológico que se caracteriza por extensa necrose e proliferação aumentada. Esses achados sublinham o potencial de dirigir intervenções terapêuticas para vias glicolíticas em TGCTs, especialmente em subtipos agressivos como os ECs. O estudo fornece uma caracterização molecular abrangente que pode informar futuras pesquisas e estratégias de tratamento para TGCTs.
