dc.descriptionA doença de Graves, e o hipertiroidismo associado, têm implicações significativas para além da própria glândula tireoide, particularmente no que diz respeito ao sistema cardiovascular. Esta revisão pretende descrever a interação complexa entre o hipertiroidismo, os mecanismos autoimunes inerentes à doença de Graves e o risco cardiovascular. Nesta revisão, sintetizamos a literatura atual sobre a doença de Graves, focando na epidemiologia, fisiopatologia, fatores genéticos e ambientais, e seu significativo impacto no sistema cardiovascular. É dada especial atenção aos mecanismos pelos quais o excesso de hormonas tiroideias e os processos autoimunes associados à doença de Graves influenciam o risco cardiovascular. O hipertiroidismo e a autoimunidade da doença de Graves contribuem para alterações na hemodinâmica cardíaca, um risco aumentado de fibrilhação auricular e inflamação crónica, com risco aumentado de desenvolvimento e progressão da aterosclerose. Estes efeitos são mediados pela ativação dos recetores das hormonas tiroideias a nível miocárdico, alterações na resistência vascular sistémica, disfunção imunológica e inflamação. A compreensão destes mecanismos é crucial para a gestão do risco cardiovascular na doença de Graves, enfatizando a necessidade de uma avaliação abrangente do risco cardiovascular e estratégias terapêuticas adaptadas a esta população.
dc.description.abstractGraves' disease, and associated hyperthyroidism, have significant implications beyond the thyroid gland itself, particularly concerning the cardiovascular system. This review aims to dissect the complex interplay between hyperthyroidism, the autoimmune mechanisms inherent to Graves' disease, and cardiovascular outcomes. We synthesize the current literature on Graves' disease in this review, focusing on epidemiology, pathophysiology, genetic and environmental factors, and its extensive impact on the cardiovascular system. Special attention is given to the mechanisms by which excess thyroid hormone and autoimmune processes associated with Graves' disease influence cardiovascular health. Excess thyroid hormones and Graves' disease autoimmunity contribute to changes in cardiac hemodynamics, an increased risk of atrial fibrillation, and chronic inflammation, thereby increasing the risk of developing and progressing atherosclerosis. These effects are mediated by the activation of thyroid hormone receptors in the heart, alterations in systemic vascular resistance, and immune dysregulation and inflammation. Understanding these mechanisms is crucial for managing cardiovascular risk in patients with Graves' disease, emphasizing the need for comprehensive cardiovascular risk assessment and tailored therapeutic strategies in this population.
