dc.descriptionO melanoma é uma neoplasia maligna com origem nos melanócitos, que são responsáveis pela produção de melanina. A maioria dos casos de melanoma (>90%) são cutâneos, sendo os melanomas mucosos e uveais menos comuns (<1-5% dos casos, com variações consoante o país). O objetivo desta revisão é rever a abordagem atual perante o melanoma cutâneo e avaliar a sua evolução nos últimos 10 anos. É também focado na implementação da terapia sistémica e na compreensão de como foi influenciada a abordagem cirúrgica. Foi efetuada uma pesquisa exaustiva utilizando a MEDLINE como base de dados e o Pubmed como o motor de busca. Como foi observado nos resultados, em geral, o tratamento cirúrgico é a abordagem preferencial nas fases iniciais do melanoma e, à medida que a doença progride para fases mais avançadas, são adotadas diferentes estratégias, como a terapia sistémica, que pode ser utilizada como tratamento neoadjuvante ou adjuvante. O desenvolvimento contínuo da imunoterapia, da "targeted therapy" e de modalidades de tratamento inovadoras oferece esperança de melhorias na sobrevivência e na qualidade de vida dos doentes com melanoma. Além disso, os avanços numa medicina de maior precisão e as abordagens baseadas em biomarcadores têm o potencial de aprimorar a seleção dos doentes e otimizar os resultados do tratamento.
dc.description.abstractMelanoma is a malignancy originating from melanocytes, which are responsible for producing melanin. Most melanoma cases (>90%) are cutaneous, with mucosal and uveal melanomas being less common (<1-5% of cases, with variations by country). The focus of this revision is to review the current management of cutaneous melanoma and assess its evolution over the last 10 years. It is also focused on the implementation of the systemic therapy and understanding how it has changed the surgical approach. A comprehensive research was conducted using MEDLINE as the database and using Pubmed as the search engine. As observed from the results, in general, surgical treatment is the preferred approach in early stages of melanoma, and as the disease progresses to later stages, different strategies are adopted, such as systemic therapy, which can be used as either neoadjuvant or adjuvant treatment. The continued development of immunotherapy molecularly targeted agents, and innovative treatment modalities offers hope for further improvements in survival and quality of life for patients with melanoma. Additionally, advancements in precision medicine and biomarker-driven approaches hold the potential to further refine patient selection and optimize treatment outcomes.
