| Title: | Human versus Artificial Intelligence: Validation of a Deep Learning Model for retinal layer and fluid segmentation in Optical Coherence Tomography Images from patients with Age-Related Macular Degeneration |

| Abstract: | Artificial intelligence (AI) models have received considerable attention in recent years for their ability to identify optical coherence tomography (OCT) biomarkers with clinical diagnostic potential and predict disease progression. This study aims to externally validate a deep learning (DL) algorithm by comparing its segmentation of retinal layers and fluid with a gold standard method of manually adjusting the automatic segmentation of the Heidelberg Spectralis HRA + OCT software. A total of sixty OCT images of healthy subjects and patients with intermediate and exudative age-related macular disease (AMD) were included. A quantitative analysis of retinal thickness and fluid area was performed and the discrepancy between the methods was investigated. The results showed a moderate to strong correlation between the metrics extracted by both software, in all groups, and an overall near-perfect area overlap was observed, except for the inner segment ellipsoid (ISE) layer. The DL system detected a significant difference in outer retinal thickness across disease stages and accurately identified fluid in exudative cases. In more diseased eyes, there was significantly more disagreement between the methods. This DL system appears to be a reliable method for accessing important OCT biomarkers in AMD. However further accuracy testing should be conducted to confirm its validity in real-world settings to ultimately aid ophthalmologists in OCT imaging management and guide timely treatment approaches. |
| Description: | Os modelos de inteligência artificial (IA) têm atraído, nos últimos anos, uma atenção considerável pela sua capacidade em identificar biomarcadores com potencial de diagnóstico clínico em imagens de tomografia de coerência ótica (OCT) e em prever progressão de doença. Este estudo pretende validar externamente um algoritmo de aprendizagem profunda (AP), comparando a sua segmentação de camadas da retina e de fluido com um método gold standard de ajuste manual da segmentação automática gerada pelo software Heidelberg Spectralis HRA + OCT. Foi incluído um total de sessenta imagens de OCT de indivíduos saudáveis e de doentes com degenerescência macular da idade (DMI) intermédia e exsudativa. Foi realizada uma análise quantitativa da espessura das camadas da retina e da área de fluido, tendo sido investigada a discrepância entre os métodos. Os resultados mostraram uma correlação moderada a forte entre as métricas extraídas por ambos os softwares, em todos os grupos, bem como uma elevada sobreposição de áreas segmentadas, exceto para a camada Inner Segment Ellipsoid (ISE). O sistema de AP detetou uma diferença significativa na espessura da retina externa entre diferentes estádios de doença e foi capaz de detetar com precisão fluido em casos exsudativos. Em estádios mais avançados de doença, foi encontrada uma significativa maior discordância entre os métodos. Este sistema de AP parece ser um método confiável para detetar e quantificar importantes biomarcadores de OCT na DMI. No entanto, futuramente, devem ser implementados outros testes de acuidade para confirmar a sua validade no contexto clínico real, onde poderá auxiliar os oftalmologistas na interpretação de imagens de OCT e orientar a abordagem terapêutica. |
