Title: Change in the trajectory of renal function after bariatric surgery

Abstract: Introduction: Obesity is known to cause health problems, but the benefits of bariatric surgery on kidney function are not clear. Long-term bariatric surgery patients require studies analyzing renal function trajectory and comparing surgical methods. Methods: In a retrospective cohort study including 735 patients, patients were stratified by glomerular filtration rate (GFR) at baseline and divided into several trajectories depending on the trajectory of their renal function. We used uni- and multivariate regression analysis to identify variables that were thought to determine change in the trajectory. Demographic, clinical, and laboratory data were collected, including preoperative and postoperative serum creatinine and urinary albumin-creatinine ratio (UACR). Statistical analysis was performed using multinomial logistic regression. Results: The study found diverse trajectories of renal function post-surgery, with significant associations between renal outcomes and factors such as age, hypertension, diabetes and metabolic syndrome. Patients with normal preoperative glomerular filtration rate (GFR) generally maintained their values post-surgery, while some patients experienced improvements in GFR. Additionally, bariatric surgery was associated with a reduction in albuminuria. Conclusion: Longitudinal monitoring of renal function is essential to understand the surgery's true impact, especially in patients with preexisting renal dysfunction. This study highlights the importance of personalized management of obese patients' comorbidities, particularly regarding renal function. bariatric surgery can positively impact renal function in obese patients, but individualized care is necessary to optimize outcomes and prevent complications.
Description: Introdução: Sabe-se que a obesidade causa problemas de saúde, mas os benefícios da cirurgia bariátrica na função renal não são claros. Doentes sujeitos a cirurgia bariátrica necessitam de estudos de longo prazo que analisem a trajetória da função renal e comparem os métodos cirúrgicos. Métodos: Em um estudo de coorte retrospetivo incluindo 735 pacientes, os pacientes foram estratificados pela taxa de filtração glomerular (TFG) no início do estudo e divididos em diversas trajetórias, dependendo da trajetória da sua função renal. Utilizamos análise de regressão uni e multivariada para identificar variáveis que se pensa determinarem mudança na trajetória. Foram recolhidos dados demográficos, clínicos e laboratoriais, incluindo creatinina sérica pré e pós-operatória e relação albumina-creatinina urinária (UACR). A análise estatística foi realizada por meio de regressão logística multinomial. Resultados: O estudo encontrou diversas trajetórias de função renal pós-operatória, com associações significativas entre desfechos renais e fatores como idade, hipertensão, diabetes e síndrome metabólica. Pacientes com taxa de filtração glomerular (TFG) pré-operatória normal geralmente mantiveram os seus valores após a cirurgia, enquanto alguns pacientes apresentaram melhorias na TFG. Além disso, a cirurgia bariátrica foi associada à redução da albuminúria. Conclusão: A monitorização longitudinal da função renal é essencial para compreender o verdadeiro impacto da cirurgia, especialmente em pacientes com disfunção renal pré-existente. Este estudo destaca a importância da abordagem personalizada das comorbilidades dos pacientes obesos, principalmente no que diz respeito à função renal. A cirurgia bariátrica pode ter um impacto positivo na função renal de pacientes obesos, mas é necessário um cuidado individualizado para otimizar os resultados e prevenir complicações.
