Change in the trajectory of renal function after bariatric surgery

Abstract

Introduction: Obesity is known to cause health problems, but the benefits of bariatric surgery on kidney function are not clear. Long-term bariatric surgery patients require studies analyzing renal function trajectory and comparing surgical methods. Methods: In a retrospective cohort study including 735 patients, patients were stratified by glomerular filtration rate (GFR) at baseline and divided into several trajectories depending on the trajectory of their renal function. We used uni- and multivariate regression analysis to identify variables that were thought to determine change in the trajectory. Demographic, clinical, and laboratory data were collected, including preoperative and postoperative serum creatinine and urinary albumin-creatinine ratio (UACR). Statistical analysis was performed using multinomial logistic regression. Results: The study found diverse trajectories of renal function post-surgery, with significant associations between renal outcomes and factors such as age, hypertension, diabetes and metabolic syndrome. Patients with normal preoperative glomerular filtration rate (GFR) generally maintained their values post-surgery, while some patients experienced improvements in GFR. Additionally, bariatric surgery was associated with a reduction in albuminuria. Conclusion: Longitudinal monitoring of renal function is essential to understand the surgery's true impact, especially in patients with preexisting renal dysfunction. This study highlights the importance of personalized management of obese patients' comorbidities, particularly regarding renal function. bariatric surgery can positively impact renal function in obese patients, but individualized care is necessary to optimize outcomes and prevent complications.
