Title: Female Genital Tract Microbiome: The Influence of Probiotics on Assisted Reproduction

Abstract: Assisted reproductive technology (ART) has been evolving since 1978, with the number of techniques performed increasing over the years. Despite continued advances, some couples continue to have difficulties getting pregnant, and it has recently been considered that the microbiome of the female genital tract (FGT) may influence embryo implantation and the establishment of pregnancy. This review aims to evaluate the role of probiotics on reproductive outcomes in infertile women on ART. A search throughout medical databases was performed, and six articles met the criteria. Five studies showed improvements in pregnancy rates, with only one demonstrating statistical significance. One article showed no improvement but reported a statistically significant reduction in the miscarriage rate in the probiotic group. Further research is needed to evaluate the true potential of probiotics, namely to assess whether they effectively modulate the FGT microbiome and if these changes are maintained over time.
Description: As técnicas de procriação medicamente assistida (PMA) têm vindo a evoluir desde 1978, tendo o número de técnicas efetuadas aumentado ao longo dos anos. Apesar dos avanços contínuos, alguns casais mantêm dificuldades em engravidar, tendo sido recentemente considerado que o microbioma do trato genital feminino (TGF) pode influenciar a implantação do embrião e o estabelecimento da gravidez. Esta revisão tem como objetivo avaliar o papel dos probióticos no sucesso reprodutivo de mulheres inférteis submetidas a PMA. Foi efetuada uma pesquisa em bases de dados médicas e seis artigos preencheram os critérios. Cinco estudos mostraram melhorias nas taxas de gravidez, sendo que apenas um demonstrou significância estatística. Um artigo não mostrou melhoria, mas observou uma redução estatisticamente significativa na taxa de aborto espontâneo no grupo dos probióticos. São necessários mais estudos para avaliar o verdadeiro potencial dos probióticos, nomeadamente para avaliar se efetivamente modulam o microbioma do TGF e se estas alterações se mantêm ao longo do tempo.
