Title: Exploring the Relationship between Bipolar Disorder and Dementia: A Narrative Review

Abstract: Background: With an increasing recognition of cognitive impairments in patients with bipolar disorder (BD), there is a growing need to explore the underlying mechanisms that may link BD to dementia. Methods: A systematic search of PubMed and Google Scholar identified English-language articles from 2001 to 2023. MeSH terms targeted BD, dementia, and related factors, screening articles based on relevance to elucidate the relationship between BD and dementia. Results: The review highlighted the cognitive effects of commonly used BD medications, noting potential dementia risks with long-term benzodiazepine and anticonvulsant use, and neuroprotective effects of lithium. It also explored shared genetic variations and neurobiological pathways underlying both BD and dementia, such as the role of the glycogen synthase kinase 3β and accelerated brain aging processes. Neuroimaging studies provided insights into structural brain changes observed in both conditions, revealing grey and white matter integrity alterations. Additionally, the review addressed medical comorbidities, lifestyle factors, and challenges accessing medical care in individuals with BD as a potential link between the two disorders. Limitations: Gaps remain in understanding neurobiological mechanisms linking BD and dementia. Furthermore, there's insufficient evidence regarding the long-term impact of pharmacological treatments on dementia risk in BD patients, necessitating further investigation. Conclusions: Understanding the shared biological mechanisms between BD and dementia is crucial for advancing clinical practice and improving patient outcomes. Targeted interventions, such as optimizing medication regimens and addressing lifestyle factors, hold promise for reducing the risk of dementia in BD patients, highlighting the clinical relevance of this research.
Description: Contexto: Com um aumento do reconhecimento da disfunção cognitiva em doentes com perturbação bipolar (PB), há uma necessidade crescente de explorar os mecanismos subjacentes que podem ligar a PB à demência. Métodos: Uma pesquisa sistemática no PubMed e Google Scholar identificou artigos em inglês de 2001 a 2023. Termos MeSH foram aplicados a PB, demência e fatores relacionados, selecionando artigos com base na relevância para elucidar a relação entre PB e demência. Resultados: A revisão destacou os efeitos cognitivos dos fármacos comumente usados na PB, observando riscos potenciais de demência com o uso prolongado de benzodiazepinas e anticonvulsivos, e os efeitos neuroprotetores do lítio. Explorou também variações genéticas compartilhadas e vias neurobiológicas subjacentes tanto à PB quanto à demência, como o papel da glicogénio sintase quinase 3β e processos de envelhecimento cerebral acelerado. Estudos de neuroimagem forneceram insights sobre mudanças estruturais cerebrais observadas em ambas as condições, revelando alterações na integridade da substância cinzenta e branca. Além disso, a revisão abordou comorbidades médicas, fatores de estilo de vida e desafios no acesso aos cuidados de saúde em indivíduos com PB como uma possível ligação entre as duas patologias. Limitações: Permanecem lacunas na compreensão dos mecanismos neurobiológicos que relacionam a PB com a demência. Além disso, há evidências insuficientes sobre o impacto a longo prazo dos tratamentos farmacológicos no risco de demência em doentes com PB, sendo necessária futura investigação. Conclusões: Compreender os mecanismos biológicos compartilhados entre a PB e a demência é crucial para otimizar a prática clínica e melhorar o prognóstico dos doentes. Intervenções direcionadas, como a otimização de regimes farmacológicos e a abordagem de fatores de estilo de vida, prometem reduzir o risco de demência em pacientes com PB, destacando a relevância clínica desta revisão narrativa.
