| Title: | Internal tremor: characterization and quantification in a cohort of neurological patients |

| Abstract: | Background: Internal tremor (IT) is an underexplored symptom. It may be present in up to 44% of patients with Parkinson's Disease (PD) and 55% of patients with essential tremor (ET). Some authors suggest a possible associated with anxiety. Objectives: We aimed to ascertain the presence of IT in a cohort of patients with PD and ET. Secondly, we intended to characterize its phenomenological features (specifically tremor frequency), to study its correlation with postural and resting tremors, and to analyze its association with anxiety. Methods: This observational cross-sectional study enrolled 50 patients diagnosed with PD or ET. Patients reporting IT completed a questionnaire regarding its characteristics, and their postural and resting tremors were quantified using a smartphone application. We further inquired whether they could mimic this sensation orally, and then recorded and analyzed their vocal imitations with acoustic analysis software. Internal tremor frequency was subsequentially calculated and compared with the patients' postural and resting tremors. Results: IT was reported in seven patients (14%). Five patients (71.4%) noted a correlation with anxiety. Five patients (71.4%) were able to characterize IT through sound, with frequencies generally falling within the range of 4-6 Hz. Conclusions: IT appears to be an infrequent phenomenon in patients with overt tremor syndromes. It could be associated with anxiety, and it can also reflect a subclinical tremor. Our study is compatible with the possibility of a central pattern generator driving the tremor observed (and felt) in patients with PD and ET. Larger studies are required to better understand IT. |
| Description: | Introdução: Tremor interno (TI) é um sintoma pouco compreendido. Pode estar presente em até 44% dos pacientes com doença de Parkinson (PD) e em até 55% dos pacientes com tremor essencial (ET). Alguns autores sugerem uma possível associação com ansiedade. Objetivos: Determinar a presença ou ausência de IT num coorte de pacientes com PD e ET. Adicionalmente, pretendemos caracterizar as suas propriedades (especificamente a frequência do tremor), estudar sua correlação com tremor postural e de repouso, e analisar sua associação com ansiedade. Métodos: Estudo observacional transversal, incluiu 50 pacientes diagnosticados com DP ou ET. Pacientes que afirmaram experienciar IT completaram um questionário sobre as suas caraterísticas, e o tremor postural e de repouso desses pacientes foi quantificado através duma aplicação de smartphone. Questionamos sobre a possibilidade de imitarem a sensação de IT oralmente e, de seguida, gravamos e analisamos o som emitido através dum software de análise acústica. A frequência do tremor interno foi calculada e comparada com o tremor postural e de repouso dos pacientes. Resultados: A presença de IT foi descrita por sete pacientes (14%). Cinco pacientes (71,4%) referiram uma associação com ansiedade. Cinco pacientes (71,4%) foram capazes de caraterizar IT através do som, com frequências geralmente entre 4-6 Hz. Conclusões: IT aparenta ser um fenómeno incomum em pacientes com doenças de componente tremórico evidente. Pode estar associado à ansiedade e também pode refletir um tremor subclínico. O nosso estudo é compatível com a hipótese da existência de um gerador de padrão central responsável pelo tremor observado (e sentido) em pacientes com PD e ET. Estudos com maior tamanho amostral são necessários para compreender melhor o IT. |
