| Title: | Tuberculosis during COVID-19 pandemic: retrospective analysis in a tertiary hospital in Portugal |

| Abstract: | Setting: Tuberculosis (TB) is a preventable and curable infectious disease with still great impact on public health. Early diagnosis and access to treatment remain critical to tackle its transmission and reduce associated mortality. COVID-19 pandemic led to considerable changes in the dynamics of medical care systems, with yet to fully determine consequences on the global fight against Tuberculosis set by the World Health Organization. Objective and Design: The present retrospective observational study aimed to characterize the clinical and epidemiological diversity of TB patients during pandemic years. Results: Between January 2020 and December 2022, 72 patients were warded and diagnosed with Tuberculosis in a tertiary hospital center in Porto, Portugal. Most were male with a median age of 54 years. Mean time to diagnosis was 95 days since onset of symptoms. Almost one third were immunosuppressed. The majority had pulmonary TB with sputum-smear positive and cavitating disease. Treatment was started on all except one patient and a larger part achieved a favourable clinical outcome. Conclusions: This report allows a perspective on the management of Tuberculosis in admitted patients while an unprecedented health crisis occurred. It paves way for additional studies to fully assess the impact of COVID-19 pandemic on Tuberculosis. |
| Description: | Introdução: A tuberculose (TB) é uma doença infeciosa prevenível e curável que mantém um grande impacto na saúde pública. O diagnóstico precoce e o acesso ao tratamento continuam a ser cruciais para combater a sua transmissão e reduzir a mortalidade associada. A pandemia de COVID-19 levou a mudanças consideráveis na dinâmica dos sistemas de cuidados médicos, com consequências ainda por determinar na luta global contra a Tuberculose, iniciada pela Organização Mundial de Saúde. Objetivo e Design: O presente estudo observacional retrospetivo visa caracterizar a diversidade clínica e epidemiológica de pacientes com tuberculose durante os anos de pandemia. Resultados: Entre janeiro de 2020 e dezembro de 2022, 72 pacientes foram admitidos e diagnosticados com tuberculose num centro hospitalar terciário no Porto, em Portugal. A maioria eram homens com uma média de idade de 54 anos. O tempo médio até o diagnóstico foi de 95 dias desde o início dos sintomas. Quase um terço eram imunossuprimidos. A maioria tinha tuberculose pulmonar com baciloscopia positiva e doença cavitada. O tratamento foi iniciado em todos, exceto um paciente, e a maioria alcançou um resultado clínico favorável. Conclusões: Esta análise permite obter uma perspetiva da gestão de Tuberculose em pacientes hospitalizados durante uma crise sem precedentes no setor da saúde. Este estabelece o moto para estudos adicionais que avaliem extensamente o impacto da pandemia por COVID-19 na Tuberculose. |
