dc.descriptionObjetivo: Determinar a taxa de luxação e avaliar os fatores de risco associados a luxação de artroplastia total da anca em doentes eletivos portugueses. Métodos: Foi realizada uma avaliação retrospetiva de todos os doentes submetidos a artroplastia total da anca eletiva num hospital central português com unidades dedicadas à anca e coluna, entre 2012 e 2017. Dados clínicos e relacionados com a artroplastia total da anca foram revistos, juntamente com registos de luxação e informação associada à cirurgia da coluna vertebral. A relação entre fatores de risco e luxação foi analisada. A análise estatística foi efetuada no SPSS (28ª versão). Resultados: 936 pacientes foram incluídos, com uma idade média de 63.27 ± 15.78 anos (66.71 ± 10.80 no grupo de luxação). A idade dos pacientes (p=0.339), género (p=0.394), IMC (p=0.989), hipertensão (p=0.952), doença cardíaca (p=0.633) e doença renal (p=0.312) não diferiram significativamente entre pacientes com e sem luxação. Diabetes foi significativamente mais prevalente em pacientes com luxação (p=0.026). A principal causa de revisão da artroplastia foi fratura periprotésica (31.75%), seguida de infeção (30.16%) e luxação (22.22%). Pacientes com episódio de luxação foram mais propensos a necessitar de cirurgia de revisão do que aqueles que sem luxação. 96.8% das luxações aconteceram nos primeiros 24 meses. Conclusão: Pacientes com diabetes parecem ter maior risco de luxação. Nenhuma outra condição mostrou correlação com a taxa de luxação. Luxação ocorre mais frequentemente nos primeiros dois anos e pacientes com instabilidade são mais propensos a passar por cirurgia de revisão. dc.description.abstractPurpose: To assess the rate of dislocation and evaluate the risk factors associated with THA dislocation in Portuguese elective patients. Methods: A retrospective assessment of all patients submitted to elective THA in a Portuguese tertiary hospital with hip and spine dedicated units, between 2012 and 2017 was performed. Clinical and THA-related data was reviewed, alongside dislocation reports and information on associated lumbar spine surgery. The relationship between risk factors and dislocation was analysed. Statistical analysis was conducted with SPSS (28th version) Results: 936 patients were included, with a mean age of 63.27 ± 15.78 (66.71 ± 10.80 in the dislocation group). Patient's age (p=0.339), gender (p=0.394), BMI (p=0.989) hypertension (p=0.952), cardiac disease (p=0.633) and renal disease (p=0.312) did not differ significantly between patients with and without dislocation. Diabetes was significantly more prevalent in patients with dislocation (p=0.026). The main cause for revision of arthroplasty was periprosthetic fracture (31.75%), followed by infection (30.16%) and dislocation (22.22%). Patients with an episode of dislocation were more likely to need a revision surgery than the ones with no dislocation. 96.8% of dislocations happened in the first 24 months. Conclusion: Patients with diabetes seem to have higher risk of dislocation. No other condition showed a significant association with dislocation rate.
