| Title: | The effect of the collateral cerebrovascular circulation on tolerance to carotid artery cross-clamping and on early outcome after carotid endarterectomy: complications. |

| Abstract: | Background: Carotid endarterectomy (CEA) is an effective surgical intervention that decreases the risk of stroke in patients with severe symptomatic carotid artery stenosis. The Circle of Willis (CoW) serves as the primary source of contralateral blood supply in situations of ipsilateral blood supply deficiency; therefore, it has been suggested that CoW's anatomy affects CEA outcomes. Objective: The aim of this study was to evaluate the associations between the cerebral collateral circulation, a positive awake test for intraoperative neurologic deficit after carotid cross-clamping (CC) under local or regional anesthesia (LRA) and postoperative adverse neurologic events. Methods: A systematic review was conducted searching MEDLINE, Cochrane and Web of Science databases for studies that assessed the cerebral circulation, including CoW variations, using neuroimaging techniques in patients who underwent carotid CC for endarterectomy under LRA. Data were extracted and analyzed systematically, as appropriate. Results: Eight eligible studies involving CC in 1313 patients, of whom 1258 underwent 1261 carotid endarterectomies, were included in the systematic review. The incidence of positive awake test among the CEA patients ranged from 4.4% to 19.7%. Overall, patients with anterior circulation insufficiency had a positive awake test ranging from 5% to 91%, while patients with posterior circulation insufficiency had a positive awake test ranging from 27% to 74%. Additionally, patients with incomplete CoW did not exhibit a statistically significant direct correlation with the presence of neurological deficits. A positive awake test in patients with contralateral carotid stenosis or occlusion ranged from 5.8% to 100%, with a meta-analytical incidence of 79.9%. Data were insufficient to evaluate the effect of the collateral circulation on early outcome after CEA. Conclusion: The presence of an incomplete CoW alone did not affect the incidence of intraoperative neurologic deficit and data were insufficient to evaluate its effect on outcomes after CEA, performed under LRA. The findings suggest that more than one abnormality of the CoW or contralateral carotid stenosis/occlusion likely predicts a positive intraoperative awake test. Additional research with consistent and internationally defined methodology is required to predict intolerance to carotid CC with the goal to consider alternative management (carotid artery stenting or medical therapy) for patients at high risk for CC intolerance. |
| Description: | Enquadramento: A endarterectomia carotídea (EC) é uma intervenção cirúrgica eficaz que diminui o risco de acidente vascular cerebral em pacientes com estenose carotídea sintomática grave. O Polígono de Willis (CoW) atua como a principal fonte de entrega de sangue contralateral em situações de deficiência na entrega de sangue ipsilateral; portanto, tem sido sugerido que a anatomia do CoW afeta os resultados da EC. Objetivo: O objetivo deste estudo foi avaliar as associações entre a circulação colateral cerebral, um awake teste positivo para défice neurológico intraoperatório após o clampe carotídeo (CC) sob anestesia local ou regional (ALR) e eventos neurológicos adversos pós-operatórios. Métodos: Foi realizada uma revisão sistemática pesquisando estudos nas bases de dados MEDLINE, Cochrane e Web of Science que avaliaram a circulação cerebral, incluindo variações do CoW, utilizando técnicas de neuroimagem em pacientes submetidos a CC carotídeo para realização de endarterectomia sob ALR. Os dados foram extraídos e analisados de forma sistemática, conforme apropriado. Resultados: Oito estudos elegíveis envolvendo CC em 1313 pacientes, dos quais 1258 foram submetidos a 1261 endarterectomias carotídeas, foram incluídos na revisão sistemática. A incidência de awake test positivo entre os pacientes submetidos a EC variou de 4,4% a 19,7%. No geral, pacientes com insuficiência na circulação anterior apresentaram um awake test positivo variando de 5% a 91%, enquanto pacientes com insuficiência na circulação posterior apresentaram um awake test positivo variando de 27% a 74%. Além disso, pacientes com CoW incompleto não apresentaram uma correlação direta estatisticamente significativa com a presença de défices neurológicos. Um awake test positivo em pacientes com estenose ou oclusão carotídea contralateral variou de 5,8% a 100%, com uma incidência meta-analítica de 79,9%. Os dados foram insuficientes para avaliar o efeito da circulação colateral no resultado precoce após EC. Conclusão: A presença de um CoW incompleto isoladamente não afetou a incidência de défice neurológico intraoperatório, e os dados foram insuficientes para avaliar o seu efeito nos resultados após EC, realizada sob ALR. Os achados sugerem que mais do que uma anormalidade do CoW ou estenose/oclusão carotídea contralateral provavelmente prevêm um awake test intraoperatório positivo. São necessárias pesquisas adicionais com metodologia consistente e internacionalmente definida para prever a intolerância ao CC carotídeo com o objetivo de considerar uma gestão alternativa (angioplastia carotídea ou terapêutica medicamentosa) para pacientes com alto risco de intolerância ao CC. |
