dc.descriptionO mieloma múltiplo (MM), a segunda neoplasia hematológica maligna mais comum, permanece incurável e a sua incidência está a aumentar. A terapia com células T do recetor de antígeno quimérico (células CAR-T) emerge como um tratamento inovador com o potencial de melhorar a sobrevida e a qualidade de vida dos pacientes com mieloma múltiplo recidivante/refratário (MMrr). Nesta revisão sistemática e meta-análise, pretendemos fornecer uma visão concisa dos mais recentes desenvolvimentos na terapia com CAR-T, avaliar as suas potenciais implicações para a prática clínica e analisar os resultados de eficácia e segurança com base na evidência mais atualizada. Uma pesquisa bibliográfica realizada de 01/01/2019 a 12/07/2023 na Medline/PubMed, Scopus e Web of Science identificou 2273 artigos, dos quais 29 preencheram os critérios especificados para inclusão na meta-análise. Os itens metodológicos para estudos não randomizados (MINORS) foram utilizados para avaliar a qualidade metodológica. Esta revisão e meta-análise seguiu as diretrizes PRISMA, com todos os componentes avaliados por dois revisores independentes. Os resultados desta meta-análise oferecem evidências robustas que apoiam a eficácia da terapia com células CAR-T no tratamento de pacientes com MMrr, com um perfil geralmente seguro e efeitos adversos controláveis. A nossa análise de subgrupos destacou a importância do tratamento prévio e do perfil de mieloma do paciente, tanto na eficácia como na segurança dos resultados.
dc.description.abstractMultiple myeloma (MM), the second most common hematologic malignance, remains incurable, and its incidence is rising. Chimeric Antigen Receptor T-cell (CAR-T cell) therapy emerges as a novel treatment with the potential to improve the survival and quality of life of patients with relapse/refractory multiple myeloma (rrMM). In this systematic review and meta-analysis, we aim to provide a concise overview of the latest developments in CAR-T therapy, assess their potential implications for clinical practice, and evaluate their efficacy and safety outcomes based on the most up-to-date evidence. A literature search conducted from 01/01/2019 to 12/07/2023 on Medline/PubMed, Scopus, and Web of Science identified 2273 articles, of which 29 fulfilled the specified criteria for inclusion in the meta-analysis. The Methodological Items for Non-Randomized Studies (MINORS) were used for evaluating the methodological quality. This comprehensive review and meta-analysis followed PRISMA guidelines, with all components evaluated by two independent reviewers. The results of this meta-analysis offer robust evidence supporting the efficacy of CAR-T cell therapy in the treatment of rrMM patients with a generally safe profile and manageable side effects. Our subgroup analysis underscored the influence of prior treatment history and the patient's myeloma profile on both effectiveness and safety outcomes.
