| Abstract: | Introduction Ductal Carcinoma in Situ (DCIS) is a precursor lesion of Invasive Breast Carcinoma (IBC), but we cannot predict which will progress to invasive disease. This study aims to define the upstaging rate and to identify upstaging predictive factors in a DCIS sample. Materials and Methods This is a retrospective study that analyzed a consecutive sample of 207 patients with a pre-operative diagnosis of DCIS treated at Centro Hospitalar Universitário São João (CHUSJ). Using descriptive statistics, we evaluated variables such as preoperative size, clinical presentation, mammography presentation and extent of lesion. A receiver operating characteristic (ROC) curve was used to quantify the diagnostic ability of the tumour size by imaging and its most suitable cut-off. Results The upstaging rate obtained was 26,2% (48/183). Only tumour size by imaging was significantly associated with DCIS upstaging. A ROC curve using the tumour size by imaging showed a cutoff at 45.5 mm as the point maximizing sensitivity and specificity (area under the curve=0.634, 95% CI: 0.545-0.724; Gini Index=0.268). Conclusion One in every four patients with an DCIS lesion on initial diagnosis will get a final diagnosis of IBC, and this finding will impact the indication for sentinel lymph node biopsy (SLNB). Upstaging was associated with the tumour size by imaging. This study is a contribution to determine which DCIS patients should undergo SLNB due to an increased risk of a final diagnosis of an invasive/microinvasive carcinoma. |
| Description: | Introdução O Carcinoma Ductal In Situ (DCIS) é uma lesão precursora do Carcinoma Invasivo da mama (IBC), mas atualmente ainda não podemos prever que lesões irão evoluir para doença invasiva. Este estudo tem como objetivo definir a taxa de upstaging e identificar fatores preditivos de upstaging numa amostra de DCIS. Materiais e Métodos Este é um estudo retrospectivo que analisou uma amostra consecutiva de 207 pacientes com diagnóstico pré-operatório de CDIS tratadas no Centro Hospitalar Universitário São João (CHUSJ). Utilizando estatísticas descritivas, avaliamos variáveis como tamanho pré-operatório, apresentação clínica, apresentação mamográfica e extensão da lesão. Uma curva característica de operação do receptor (ROC) foi usada para quantificar a capacidade diagnóstica do tamanho do tumor por imagem e seu ponto de corte mais adequado. Resultados A taxa de upstaging obtida foi de 26,2% (48/183). Apenas o tamanho do tumor por imagem estava significativamente associado ao upstaging de DCIS. Uma curva ROC usando o tamanho do tumor por imagem mostrou um ponto de corte em 45,5 mm como o ponto de máxima sensibilidade e especificidade (área sob a curva = 0,634, IC 95%: 0,545-0,724; Índice Gini = 0,268). Conclusão Uma em cada quatro pacientes com uma lesão de DCIS no diagnóstico inicial receberá um diagnóstico final de IBC, e este achado influenciará a indicação para biópsia do gânglio sentinela (SLNB). O upstaging foi associado ao tamanho do tumor por imagem. Este estudo é uma contribuição para determinar quais as pacientes com DCIS que devem ser submetidos a SLNB devido a um risco aumentado de um diagnóstico final de carcinoma invasivo/microinvasivo. |
