| Title: | Association between placental pathology and early-onset fetal growth restriction: a systematic review |

| Abstract: | Objective: Fetal growth restriction (FGR) is defined as the failure of the fetus to achieve its genetically determined growth potential, mainly caused by placental dysfunction. Previous studies have shown that early- and late-onset FGR could have a different histopathology background. Our aim is to compare the placental lesions present in early-onset fetal growth restriction with that of late-onset FGR. Study design: We performed a systematic review according to the PRISMA guideline. Observational studies, only in singleton pregnancies, evaluating the association between fetal growth restriction and placental lesions in early- versus late-onset FGR were included. A qualitative analysis was undertaken. The risk of bias was assessed using the NIH Study Quality Assessment Tool. Results: We included six articles. All studies showed a higher rate of maternal vascular malperfusion (MVM) lesions in the early-onset FGR groups when compared to late-onset ones. Only one study reported that fetal vascular malperfusion (FVM) and villitis of unknown etiology (VUE) were higher in early-onset FGR. Two studies found that fetal/placental weight ratio is lower in the early-onset group. Five articles reported that early-onset FGR is often associated with pre-eclampsia. Conclusion: This review shows that early-onset FGR cases are associated with specific placental histopathology, such as maternal vascular malperfusion lesions. Future studies should explore the role of the placenta on the pathogenesis of FGR. Placental histopathological examination is important to better understand the pathophysiology of FGR and can contribute to a better management of FGR. The study of placenta is useful to determine the risk of recurrence in subsequent pregnancies. |
| Description: | Objetivo: A restrição de crescimento fetal (RCF) é definida pela incapacidade do feto em atingir o potencial de crescimento geneticamente determinado, sendo causada principalmente por disfunção placentária. Estudos anteriores demonstraram que a RCF precoce e a RCF tardia podem ter alterações histopatológicas diferentes. O objetivo deste estudo é comparar as lesões placentárias presentes na RCF de início precoce com as existentes na RCF de início tardio. Métodos: Foi realizada uma revisão sistemática de acordo com as recomendações PRISMA. Foram incluídos estudos observacionais, apenas em gestações únicas, que avaliaram a associação entre restrição de crescimento fetal e lesões placentárias na RCF precoce versus na RCF tardia. Foi efetuada uma análise qualitativa dos resultados. O risco de viés foi avaliado através da NIH Study Quality Assessment Tool. Resultados: Foram incluídos seis artigos. Todos os estudos demonstraram uma frequência mais elevada de lesões de má perfusão vascular materna nos grupos de RCF precoce quando comparado com os grupos de RCF tardia. Apenas um estudo relatou que lesões de má perfusão vascular fetal e vilite de etiologia desconhecida foram mais frequentes na RCF de início precoce. Dois estudos concluíram que o rácio peso fetal/peso placentário foi inferior nos casos de RCF precoce. Cinco artigos afirmam que a RCF de início precoce está frequentemente associada a pré-eclâmpsia. Conclusão: Esta revisão sistemática demonstra que os casos de restrição de crescimento fetal precoce estão associados a alterações histopatológicas placentárias específicas, tais como lesões de má perfusão vascular materna. Estudos futuros deverão explorar o papel da placenta no desenvolvimento de restrição de crescimento fetal. O estudo anatomopatológico da placenta contribui para um melhor conhecimento da fisiopatologia e desse modo pode ajudar a definir protocolos de orientação nos casos de RCF. É também útil para estabelecer o risco de recorrência em gestações futuras. |
