| Title: | PETTICOAT for acute type B aortic dissection: a systematic review |

| Abstract: | INTRODUCTIONː The provisional extension to induce complete attachment (PETTICOAT) technique was developed as an attempt to deal with the limitations of thoracic endovascular aortic repair (TEVAR) in the treatment of acute type B aortic dissection (TBAD). However, the benefit of PETTICOAT for TBAD remains unclear. EVIDENCE ACQUISITIONː A systematic review of literature on TBAD managed with the PETTICOAT technique was conducted to assess outcomes and aortic remodelling. EVIDENCE SYNTHESISː Eleven studies were included with a total of 360 patients. All patients had complicated TBAD. Mean technical success rate was 99.7%. Mean last-available FU date was 21.6 months. Mortality was 0.5% perioperatively, 0.9% at 30-day, 4.4% at 12-month and 9.5% at last FU. Rupture happened in 1.9% perioperatively and at 30-day, 2.3% at 12-month and 2.4% at last FU. Spinal cord ischemia developed in 0.5% perioperatively, 0.9% at 30-day, 0% at 12-month and 0.7% at last FU. Peripheral malperfusion occurred in 0.5% perioperatively and at 30-day and none at 12-month and last FU. Acute renal failure happened in 2.2% perioperatively, 2.9% at 30-day and none at 12-month and last FU. There were no cases of stent fracture, stent misalignment, device migration, penetrating aortic ulcer or stent graft-induced new entry. CONCLUSIONSː Mortality after PETTICOAT was lower than TEVAR and open surgery. Aortic rupture and clinical complications had similar rates to TEVAR. Technical success was high. There was favourable aortic remodelling, particularly in patients presenting with patent and complete FL thrombosis, but no comparison could be made with TEVAR due to heterogeneity between reports. |
| Description: | INTRODUÇÃO: A técnica "provisional extension to induce complete attachment" (PETTICOAT) foi desenvolvida como uma tentativa de abordar as limitações da técnica "thoracic endovascular aortic repair" (TEVAR) no tratamento da disseção aórtica aguda do tipo B. No entanto, o benefício da técnica de PETTICOAT para o tratamento da disseção aórtica aguda do tipo B permanece por esclarecer. AQUISIÇÃO DE EVIDÊNCIA: Foi elaborada uma revisão sistemática da literatura sobre disseção aórtica aguda do tipo B abordada com a técnica de PETTICOAT, para avaliar "outcomes" e "remodelling" aórtico. SÍNTESE DE EVIDÊNCIA: Foram incluídos onze estudos, com um total de 360 doentes. Todos os doentes apresentaram disseção aórtica aguda do tipo B. O sucesso técnico médio foi de 99.7%. A data de último "follow-up" ocorreu em média aos 21.6 meses. A mortalidade foi de 0.5% no perioperatório, 0.9% aos 30 dias, 4.4% aos 12 meses e 9.5% no último "follow-up". Rotura aórtica ocorreu em 1.9% no perioperatório e aos 30 dias, 2.3% aos 12 meses e 2.4% no último "follow-up". Isquemia medular desenvolveu-se em 0.5% no perioperatório, 0.9% aos 30 dias, 0% aos 12 meses e 0.7% no último "follow-up". Malperfusão periférica ocorreu em 0.5% no perioperatório e aos 30 dias e não houve casos aos 12 meses ou no último "follow-up". Falência renal aguda verificou-se em 2.2% no perioperatório, 2.9% aos 30 dias e não houve casos aos 12 meses ou no último "follow-up". Não houve casos de fratura de "stent", "misalignment" de "stent", migração de componentes protésicos, úlcera aórtica penetrante ou "stent graft-induced new entry". CONCLUSÕES: A mortalidade após aplicação da técnica PETTICOAT foi inferior à verificada com TEVAR e cirurgia aberta. Rotura aórtica e complicações clínicas tiveram percentagens semelhantes à técnica TEVAR. O sucesso técnico foi elevado. Verificou-se um "remodelling" aórtico favorável, particularmente nos doentes que se apresentaram com falso lúmen patente ou completamente trombosado; no entanto, não foi possível comparar estes resultados com a técnica TEVAR devido à heterogeneidade entre estudos. |
