Title: Anxiety and burnout in portuguese medical students in the final year of medical school: A narrative review

Abstract: Purpose: Mental health has been increasingly addressed in medical students, with high percentages of anxiety and burnout noted. Due to the known consequences for individuals themselves and their patients, evaluating the impact of the final year of medical school on the well-being of these students is essential. This period is atypical, with increased demands and higher levels of pressure. The aim of this study was to synthesize data on anxiety and burnout levels in medical students in their final year, emphasizing levels among Portuguese medical students in their final (6th year) of study, as well as identifying potential contributing factors and areas of interest for future research in this field. Method: A search was conducted in four databases (PubMed, Scopus, Web of Science Core Collection, and PsycInfo) using the following search terms: "(last year" OR "final year") AND (anxiety OR burnout) AND (medical students)", along with a filter for studies involving the Portuguese population: "(anxiety OR burnout) AND (medical students) AND (Portugal)". Results: Following the application of the inclusion and exclusion criteria, the search yielded 14 publications referring, predominantly, to observational and cross-sectional studies. Final-year medical students exhibited generally high levels of stress and anxiety. However, there is a scarcity of studies on anxiety and, specially, addressing burnout in final year medical students. Burnout was detected in 27.6% of final-year medical students in Cyprus. Conclusion: More high-quality research about anxiety and burnout in final-year medical students is warranted, preferably employing longitudinal designs and multi-institutional settings. The elevated levels of anxiety and burnout observed in some studies underscore the necessity for the implementation of cost-effective prevention methods, to ensure that newly-arrived professionals are equipped to deliver optimal care.
Description: Introdução: A saúde mental tem sido cada vez mais abordada em estudantes de Medicina, sendo notadas elevadas percentagens de ansiedade e burnout. Pelas consequências conhecidas, para os próprios indivíduos e para os seus utentes, revela-se essencial uma avaliação do impacto do último ano do curso no bem-estar destes estudantes, por se tratar de um período atípico, com mais exigências e com um maior nível de pressão. O objetivo deste estudo foi sintetizar os dados relativos aos níveis de ansiedade e burnout nos estudantes de Medicina no último ano do curso, enfatizando os níveis nos estudantes portugueses; e quais os fatores potenciadores e as áreas que possam ter interesse para investigações futuras neste âmbito. Métodos: Foi realizada uma pesquisa bibliográfica em quatro bases de dados (PubMed, Scopus, Web of Science Core Collection e PsycInfo), com os seguintes termos de pesquisa: "("last year" OR "final year") AND (anxiety OR burnout) AND (medical students)" e, associadamente, uma filtragem de estudos na população portuguesa: "(anxiety OR burnout) AND (medical students) AND (Portugal)". Resultados: Após aplicação dos critérios de inclusão e exclusão, a pesquisa resultou em 14 publicações maioritariamente de caráter observacional e transversal. Na bibliografia internacional, os estudantes de Medicina do último ano do curso apresentaram elevados níveis de stress e ansiedade. Há ainda poucos estudos sobre a ansiedade e, especialmente, sobre o burnout, em estudantes do último ano do curso de Medicina. No Chipre, detetou-se burnout em 27,6% dos alunos do último ano do curso. Conclusão: Revela-se necessário o aumento de investigação de qualidade relativamente a ansiedade e burnout nos estudantes de Medicina do último ano do curso, preferencialmente com métodos longitudinais e contextos multi-institucionais. Os elevados níveis de ansiedade e burnout observados em alguns estudos apontam para a necessidade de implementação de métodos de prevenção custo-efetivos, de modo a garantir recém-profissionais em plena capacidade na prestação de cuidados.
