| Abstract: | Talking about death remains difficult for many health care professionals, both with the patient and family, and among the team. In palliative care, talking about death should not be limited to communicating bad news to the patient and/or the family (that the patient has died, or is at the end of life). It is also important to talk about death during this process of finitude with the patient and the family so that they can make informed decisions, leave a legacy, share fears, emotions, and so on. It is crucial to care for the patient in all dimensions and can be a facilitator for the family's appropriate bereavement. Palliative Care teams must be specialized and trained for good communication and the importance that as well as the physical dimension it is extremely important to care for the psychological, social and spiritual dimensions, which includes talking about death, yet even then difficulties arise. How will it be, then, with non-specialized teams but that have in their units patients in Palliative Care? We are referring to the Integrated Continued Care Units (UCCI) that, belonging to the National Network of Integrated Continued Care (RNCCI), have in their institutions (public or private), patients with Palliative Care needs that have already been referred or derived from a worsening of the clinical picture of patients already hospitalized. In the UCCI, the professionals do not have specific formation in Palliative Care, so the intervention passes through constant articulation with the intra-hospital Palliative Care teams of the region. This articulation is more of a medicinal nature, for comfort care, through the relief of physical pain, not attending, therefore, to the other dimensions, which are the responsibility of the health professionals of the institution where the patient is hospitalized. Thus, for the professionals of these Units (UCCI), talking about death and its process, both with the patient and the family, should be an added difficulty, besides taking care of all dimensions of the human being in this phase of life. Thus, it is pertinent to understand which factors may facilitate and/or hinder talking about death and its process, from the perspective of health professionals in an Integrated Long-term Care Unit. Thus, the aim of this study is, through the application of semi-structured interviews to health professionals of a UCCI located in Vila Nova de Gaia, to identify the difficulties mentioned by them, which makes it difficult to talk about death with patients, with the family and among the team. On the other hand, we also identified the factors that facilitate talking about death and the strategies used, both communicational and emotional. |
| Description: | Falar sobre a morte continua a ser difícil para muitos profissionais de saúde, quer com o paciente e a família, quer entre a equipa. Em Cuidados Paliativos, o falar sobre a morte não se deve cingir à comunicação de más notícias ao paciente e/ou à família (que o paciente morreu, ou está em final de vida). Importa também falar sobre a morte durante esse processo de finitude com o paciente e com a família para poderem tomar decisões informadas, deixar um legado, partilharem medos, emoções, etc.. É crucial cuidar do paciente em todas as suas dimensões, podendo ser um elemento facilitador para o luto apropriado da família. As equipas de Cuidados Paliativos devem ser especializadas e formadas para uma boa comunicação e para a importância de que, tanto quanto a dimensão física é extremamente importante cuidar das dimensões psicológica, social e espiritual, o que inclui falar sobre a morte, no entanto, mesmo assim, surgem dificuldades. Como será, então, com equipas não especializadas mas que têm nas suas unidades pacientes em Cuidados Paliativos? Referimo-nos às Unidades de Cuidados Continuados Integrados (UCCI) que pertencendo à Rede Nacional de Cuidados Continuados Integrados (RNCCI), têm nas suas instituições (públicas ou privadas), pacientes com necessidades de Cuidados Paliativos que já vêm referenciados ou derivado de um agravamento do quadro clínico de pacientes já internados. Nas UCCI, os profissionais não têm formação específica em Cuidados Paliativos, pelo que a intervenção passa pela articulação constante com as equipas de Cuidados Paliativos intra-hospitalares da região. Esta articulação é mais do foro medicinal, para cuidados de conforto, através do alívio da dor física, não atendendo, por isso, às outras dimensões, que ficam a cargo dos profissionais de saúde da Instituição onde o paciente se encontra internado. Desta forma, para os profissionais destas Unidades (UCCI), falar sobre a morte e o seu processo, quer com o doente, quer com a família, deverá ser uma dificuldade acrescida, para além de cuidarem de todas as dimensões do ser humano nesta fase da vida. Considera-se assim pertinente entender que fatores poderão facilitar e/ou dificultar o falar sobre a morte e todo o seu processo, na perspetiva do profissional de saúde de uma Unidade de Cuidados Continuados Integrados. Assim, o objetivo deste estudo é, através da aplicação de entrevistas semiestruturadas a profissionais de saúde de uma UCCI localizada em Vila Nova de Gaia, identificar as dificuldades referidas pelos mesmos, que os faz ter dificuldade em falar sobre a morte com os pacientes, com a família e entre a equipa. Por outro lado identificar também, que factores se encontram como facilitadores para falarem sobre a morte e que estratégias utilizam quer comunicacionais, quer emocionais. |
