Title: Esófago de Barrett: tratamento endoscópico versus tratamento cirúrgico - uma revisão sistemática

Abstract: Introduction: Barrett's esophagus is an alteration of the epithelial lining of the distal esophagus, associated with gastroesophageal reflux, in which the squamous epithelium is replaced by another cylindrical type, with intestinal differentiation. Objectives: The aim of this systematic review is to analyze the effects of endoscopic versus surgical treatment on the eradication and recurrence of Barrett's Esophagus. Methods: According to the Prisma 2020 protocol, a bibliographic search was made from November 2022 to January 2023 in the PUBMED, Web of Sciences, SCOPUS, LILACS and Google Scholar databases, using the following keywords as the term MESH: "barrett's esophagus/endoscopic treatment AND barrett's esophagus/antireflux surgery". In total, 476 articles were found and the inclusion and exclusion criteria were applied. Results: The search resulted in a total of 476 articles (Pubmed - 366 articles, Web of Sciences - 39, SCOPUS - 61, Lilacs - 5 articles; Google Scholar - 5 articles). 11 non-randomized studies were included, totaling 959 patients. The highest rate of eradication of metaplasia and dysplasia obtained with endoscopic treatment was 93% and 96%, respectively, versus 41% with surgical treatment. Combining the two treatments resulted in a rate of 53 to 55.6%. As for recurrence, the highest rate with endoscopic treatment was 30.3% versus 10% with surgical treatment. Combining the two treatments resulted in a rate of 30.7% to 33%. Discussion/Conclusion: Both endoscopic and surgical treatment are safe options for treating Barrett's esophagus. The difference between the populations and the absence of reports referring to some data made analysis and comparison difficult, making it impossible to determine the treatment with the greatest benefit and effectiveness. It is necessary to carry out more controlled and randomized clinical trials with a larger number of patients and longer monitoring times in order to evaluate and compare the effects after endoscopic and surgical treatment.
Description: Introdução: O esófago de Barrett é uma alteração do revestimento epitelial do esófago distal, associado ao refluxo gastroesofágico, na qual o epitélio pavimentoso é substituído por outro de tipo cilíndrico, com diferenciação intestinal. Objetivos: O objetivo desta revisão sistemática é analisar os efeitos do tratamento endoscópico versus cirúrgico na erradicação e recorrência do Esófago de Barrett. Métodos: De acordo com o protocolo Prisma 2020, foi realizada uma pesquisa bibliográfica, de novembro de 2022 a janeiro de 2023 nas bases de dados PUBMED, Web of Sciences, SCOPUS, LILACS e Google Schoolar, usando as seguintes palavras-chaves como termo MESH: "barrett's esophagus/endoscopic treatment AND barrett's esophagus/antireflux surgery". No total, foram encontrados 476 artigos e foram aplicados os critérios de inclusão e de exclusão. Resultados: A pesquisa resultou no total de 476 artigos (Pubmed - 366 artigos, Web of Sciences - 39, SCOPUS - 61, Lilacs - 5 artigos; Google Scholar - 5 artigos). Foram incluídos 11 estudos não randomizados, totalizando 959 doentes. A maior taxa de erradicação de metaplasia e displasia, obtida no tratamento endoscópico, foi de 93% e 96%, respetivamente, versus 41% do tratamento cirúrgico. A combinação dos dois tratamentos resultou numa taxa de 53 a 55.6%. Quanto à recorrência, a maior taxa no tratamento endoscópico foi de 30.3% versus 10% do tratamento cirúrgico. A combinação dos dois tratamentos resultou numa taxa de 30.7% a 33%. Discussão/ Conclusão: Tanto o tratamento endoscópico como o tratamento cirúrgico são opções seguras para o tratamento do esófago de Barrett. A diferença entre as populações e a ausência de relatos alusivos a alguns dados dificultaram a análise e a comparação, não sendo possível determinar o tratamento com maior benefício e eficácia. É necessário a realização de mais ensaios clínicos controlados e randomizados com maior número de doentes e maior tempo de monitorização a fim de avaliar e comparar os efeitos após o tratamento endoscópico e cirúrgico.
