| Title: | Aortic valve surgery with stented versus stentless bioprosthesis in patients with active infective endocarditis |

| Abstract: | Objectives: compare clinical and hemodynamic outcomes after aortic valve replacement (AVR) with a stented (Trifecta-TF) versus a stentless bioprosthesis (Freedom Solo or Solo Smart-FS) in patients with active infective endocarditis (IE). Methods: single-center, retrospective, observational and comparative study including active IE patients who underwent AVR with TF or FS between June 2009 and December 2019. Survival and re-intervention were checked in December 2022. Chi-square or Fisher and Mann-Whitney tests were used for groups comparison and Kaplan-Meier method for time-to-event outcomes. Results: Sixty-eight patients with active IE (21 FS and 47 TF) were included. The median age of FS patients was 68 vs. 69 years in TF (p=0.253). There were 66.7% male patients in the FS vs. 72.3% TF (p=0.508). Median EuroSCORE II was 9.4 (1.0 to 50.8) in FS vs. 16.7 (1.7 to 91.0) in TF (p=0.353). Hospital mortality occurred in 9.5% FS and 25.5% TF (p=0.20). After excluding these patients, the 1-, 5- and 8-year cumulative survival rates for FS vs. TF were 89.5% vs. 88.6%, 84.2% vs. 77.1%, 60.5% vs. 59.1%, respectively (Log-rank p=0.92). There was 1 reoperation in FS at 6 years of follow-up due to structural valve deterioration; and 6 in TF (5 due to endocarditis at 1-, 3-, 5- (n=2) and 11-years of follow-up and 1 due to nonstructural valve deterioration 6-years after AVR:10-years freedom-from-reoperation of 90.9% vs. 80.8%, respectively (Log-rank, p=0.087). Conclusions: TF bioprosthesis was implanted in higher risk patients. Long-term overall survival was similar in stented and stentless aortic bioprosthesis, but FS patients presented higher freedom from prosthesis related reoperation. |
| Description: | Objetivos: comparar os resultados clínicos e hemodinâmicos após substituição valvular aórtica (SVA) com biopróteses com stent (Trifecta-TF) vs sem stent (Freedom Solo ou Solo Smart-FS) em doentes com endocardite infeciosa (EI) ativa. Métodos: Estudo retrospetivo, observacional e comparativo de centro único, incluindo doentes com EI ativa que realizaram SVA com TF ou FS entre junho de 2009 e dezembro de 2019. A sobrevivência e necessidade de reintervenção foram verificadas em dezembro de 2022. Foram utilizados testes de Qui-Quadrado ou Fisher e Mann-Whitney para comparação de grupos e o método de Kaplan-Meier para estudar as variáveis de tempo até ao evento. Resultados: Foram incluídos 68 doentes com EI ativa (21 FS e 47 TF). A idade média dos doentes FS foi de 68 anos vs. 69 anos nos doentes TF (p=0,253). A maioria dos doentes era do sexo masculino em ambos os grupos (66,7% FS vs 72,3%, p = 0,508). A mediana do EuroSCORE II foi de 9,4 (1,0 a 50,8) no grupo FS vs 16,7 (1,7 a 91,0) no grupo TF (p=0,353). A mortalidade hospitalar ocorreu em 9,5% FS e 25,5% TF (p=0,20). Após a exclusão destes doentes, as sobrevivências cumulativas aos 1, 5 e 8 anos para FS vs TF foram de 89,5% vs 88,6%, 84,2% vs 77,1%, 60,5% vs 59,1%, respetivamente (Log-rank p=0,92). Verificou-se 1 reoperação FS aos 6 anos de seguimento ¬devido a deterioração estrutural da prótese; e 6 TF (5 devido a endocardite aos 1, 3, 5 (n = 2) e 11 anos de follow-up e 1 devido a deterioração não estrutural da prótese aos 6 anos após a SVA). A taxa de livre de reoperação relacionada com a bioprótese aos 10 anos foi de 90,9% FS vs 80,8% TF, respetivamente (Log-rank, p=0,087). Conclusões: A prótese biológica com TF foi implantada em doentes de maior risco. A sobrevivência a longo prazo foi semelhante nos doentes com próteses aórticas com stent e sem stent, mas os doentes FS apresentaram maior tempo livre de reoperação relacionada com a prótese. |
