| Title: | Hemophilia and Acute Myocardial Infarction: a Systematic Review |

| Abstract: | Introduction: hemophilia is a coagulopathy where there is an absence or deficit of factors VIII or IX of the coagulation cascade, leading to high bleeding risk. In the past, people with hemophilia (PWH) had a markedly shorter life expectancy than the general population (GP). Over time, developments in pharmacotherapy and overall care have led to an increase in life expectancy. Given these, PWH could be at a higher risk of developing age-related diseases, such as acute myocardial infarction (AMI). Notably, the coagulation cascade plays an important role in the physiopathology of AMI. As such, there is interest in ascertaining if PWH could be at lower risk of AMI. Given this background, this study aimed to determine the prevalence of AMI in PWH, as compared to the GP, and the risk of mortality of AMI. Evidence acquisition: a search was performed on the Medline (PubMed), Scopus and ISI Web of Knowledge databases using the queries "hemophilia AND coronary artery disease", "hemophilia AND ischemic heart disease", "hemophilia AND myocardial infarction". Articles where the prevalence/mortality of AMI was described were included, whereas mechanistic and animal studies, and those with less than ten subjects were excluded. Evidence synthesis: eight articles reported a prevalence of AMI lower or without significant differences between in PWH and the GP. Three of the nine articles concerning mortality of AMI reported a lower mortality in PWH, and none reported a higher mortality. Conclusions: studies suggest that PWH have a lower prevalence (and mortality) of AMI than the GP. |
| Description: | Introdução: a hemofilia é uma coagulopatia na qual existe ausência ou défice dos fatores VIII ou IX da cascata da coagulação, cursando com elevado risco hemorrágico. No passado, pessoas com hemofilia (PCH) apresentavam uma esperança média de vida marcadamente mais curta que a população geral (PG). Ao longo do tempo, desenvolvimentos na farmacoterapia e nos cuidados de saúde gerais levaram ao aumento na esperança média de vida. Desta forma, PCH poderão estar sob um mais elevado risco de desenvolverem doenças relacionadas com a idade, como o enfarte agudo do miocárdio (EAM). Notavelmente, a cascata da coagulação desempenha um papel importante na fisiopatologia do EAM. Assim, existe interesse em averiguar se PCH poderão estar sob um menor risco de EAM. Tendo em conta esta informação, este estudo destinou-se a determinar a prevalência de EAM em PCH, comparando-a com a PG, e o risco de mortalidade de EAM. Aquisição de evidência: realizou-se uma pesquisa nas bases de dados Medline (PubMed), Scopus and ISI Web of Knowledge usando as queries "hemophilia AND coronary artery disease", "hemophilia AND ischemic heart disease", "hemophilia AND myocardial infarction". Artigo nos quais a prevalência/mortalidade de EAM foi descrita foram incluídos, ao passo que estudos mecanísticos e em animais, e aqueles com menos de 10 participantes foram excluídos. Síntese de evidência: oito artigos reportavam uma prevalência de EAM menor ou sem diferenças estatisticamente significativas entre PCH e a PG. Três dos nove artigos acerca da mortalidade de EAM reportaram uma menor mortalidade em PCH, e nenhum reportou mortalidade maior. Conclusões: estudos sugerem que PCH apresentam uma menor prevalência (e mortalidade) de EAM do que a PG. |
