Title: Early Multimodal Neuromonitoring Parameters as Prognostic Factors For Critically Ill Spontaneous ICH Patients

Abstract: Background: Critically ill patients with spontaneous intracerebral hemorrhage (ICH) warrant a recognizable need for further knowledge on stratification, outcome prediction, neurocritical care and early therapeutic withdrawal. Our goal was to analyze the association between early neuromonitoring signals, from the first 48 hours, in ICH patients with an optimal level of care, and six-month functional status. Methods: This is a single-center retrospective observational study. Neuromonitoring signal records and clinical data from ICH patients, admitted to a Neurocritical Care Unit (NCCU), between January 2015 and September 2021 were considered for analysis. Eligibility criteria included adult age, confirmation of ICH by neuroimaging and ≥ 24 hours of neuromonitoring. Six-month functional status was assessed by modified Rankin Scale (mRS) and dichotomized as a cut-off of severe outcome (mRS 5 and 6). Results: A total sample of 65 patients, mostly males with a mean age of 62.6 years, was analysed. During the first 48 hours of NCCU admission, mean Intracranial Pressure (ICP) had no statistically significant difference between groups (p=0.258). Mean cerebrovascular reactivity pressure index (PRx) was significantly higher in the group with the worst outcome (p=0.018), indicating impaired autoregulation. Other PRx-related variables, namely % of time over 0.2 and those also differed significantly between these groups. Mean Cerebral Perfusion Pressure (CPP) was significantly higher in the group with mRS 1-4 (p=0.029), but the difference between real CPP and optimal CPP was higher in the group with the worst outcome (p=0.023). The difference between real CPP and optimal CPP was the neuromonitoring signal which sustained its significance in multivariable analysis when controlled for other factors. Conclusions: ICP probe placement in ICH patients can provide information as well as therapeutic targets. CPP and PRx are two neuromonitoring parameters that may help predict, in the first 48 hours, the functional outcome of critically ill patients with ICH.
Description: Introdução: Pacientes com hemorragia intracerebral (HIC) espontânea têm uma levada morbilidade e mortalidade. As diretrizes atuais apoiam a admissão destes nos cuidados neurocríticos, para monitorização cerebral contínua e para tratamento precoce. O objetivo deste estudo é determinar o estado funcional após 48h de neuromonitorização, nos primeiros seis meses. Métodos: Representa um estudo observacional retrospetivo unicêntrico. Todos os doentes admitidos na Unidade de Cuidados Neurocríticos, entre janeiro de 2015 e setembro de 2021, com o diagnóstico de HIC. Acedeu-se à neuromonitorização e aos dados clínicos. Definiu-se como critérios de elegibilidade ser adulto, ter diagnóstico de hemorragia intracerebral, confirmado por neuroimagem e ter, pelo menos, 24 horas de neuromonitorização. O estado funcional aos seis meses avaliou-se através da modified Rankin Scale (mRS), dicotomizando-se em dois grupos, um deles com maior grau de disfunção, mRS 5-6. Resultados: Uma amostra total de 65 pacientes foi analisada, sendo a maioria do sexo masculino e com média de idades de 62,6 anos. Durante as primeiras 48 horas de internamento, a análise da pressão intracraniana (PIC) média não apresentou diferença estatisticamente significativa entre os dois grupos (p=0,258). Contudo, o índice de pressão da reatividade cerebrovascular (PRx) foi significativamente maior no grupo com maior grau de disfunção (p=0,018), indicando uma pior autorregulação. Outras variáveis relacionadas com PRx, % de tempo acima de 0,2 e a dose, também diferiram significativamente entre os dois grupos. A pressão média de perfusão cerebral (CPP) foi significativamente maior no grupo com mRS 1-4 (p=0,029), mas, a diferença entre a CPP real e a CPP ótima foi maior no grupo com pior estado funcional (p=0,023). A diferença entre a CPP real e CPP ótima foi o sinal que manteve significância na análise multivariada, quando controlado por outros fatores. Conclusões: A monitorização contínua da PIC pode fornecer informação útil nestes doentes, como, orientar possíveis alvos terapêuticos. Para além disso, fornece também informação sobre o valor da CPP e do PRx, mostrando que análise das primeiras 48 horas, pode ajudar a prever o estado funcional destes doentes críticos.
