| Title: | Is Fetal Sex a Determiner of Placental Weight? A Systematic Review |

| Abstract: | Overview and aim: The placenta is known for its key role in mediating interactions between the fetus and the mother. A deviation in the weight of the placenta, specifically when disproportionate to the birthweight, has been associated with many cardiovascular and metabolic diseases. Evidence has also found these diseases to be impacted by fetal sex. Such findings seem to deserve more scrutiny, so our aim was to assess how fetal sex may influence placental weight. Study Design and Methods: A systematic review was carried out by searching in PubMed and Web of Science databases, between the 15th and the 23rd of December of 2022, using the query: "fetal sex AND placental weight", and filtering for "humans" and for languages "English" and "Portuguese". Additionally, the reference list of chosen articles was screened for other relevant studies. Studies were eligible if they correlated placental weight with fetal sex. Systematic reviews, letters to editors and articles that assessed placental weight according to fetal sex only in pathological cases, both in the mother or the fetus, were ruled out. The selected articles quality and internal validity was evaluated with National Intitutes of Health Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies. A thorough qualitative analysis of the results was conducted. Results: A total of eleven articles were included in this review, with an overall 594 441 deliveries being accounted for. Four studies reported a higher placental weight in male fetuses with statistical significance. Three have not found it statistically different, even though one admitted that a larger cohort could provide significance. Other four articles have not reported this analysis, but, of these, two mentioned a tendency for a higher placental weight in male fetuses. Discussion: A heavier placenta in male fetuses is linked to an increased efficiency in growth of the fetus and consequently of their birthweight. However, this investment in "quantitative" growth rather than in a "qualitative" one, will ultimately compromise not only their gestation, but also their adult-life health. |
| Description: | Perspetiva geral e objetivo: A placenta é conhecida pelo seu papel chave na mediação de interações entre o feto e a mãe. Um desvio no peso da placenta, especificamente quando desproporcional ao peso ao nascimento, tem sido associado a doenças cardiovasculares e metabólicas. Estas têm também sido relacionadas com o sexo fetal. Tais resultados merecem maior escrutínio, sendo o nosso objetivo avaliar como o sexo fetal pode influenciar o peso da placenta. Desenho de Estudo e Métodos: Foi efetuada uma revisão sistemática, iniciada pela pesquisa nas bases de dados PubMed e Web of Science, entre 15 e 23 de dezembro de 2022, usando a query "fetal sex AND placental weight", filtrando para "humanos" e para as línguas "Inglês" e "Português". Adicionalmente, a lista de referências dos artigos selecionados foi analisada em busca de outros estudos de relevo. Os estudos eram elegíveis se correlacionassem o peso da placenta com o sexo fetal. Revisões sistemáticas, cartas aos editores e artigos que avaliam o peso da placenta de acordo com o sexo fetal, apenas em caso de patologia da mãe ou do feto, foram excluídos. A qualidade e validade interna dos artigos selecionados foi analisada com a Ferramenta de Avaliação da Qualidade de Coortes Observacionais e Estudos Cross-Sectional do National Institutes of Health. Foi conduzida uma análise qualitativa dos resultados. Resultados: Onze artigos foram incluídos nesta revisão, contando com 594 441 partos. Quatro estudos reportaram um peso da placenta superior nos fetos do sexo masculino com significância estatística. Três não encontraram esta diferença estatística, contudo um destes admite a possibilidade de numa coorte maior poder verificar-se esta significância. Outros quatros artigos não reportaram terem realizado esta análise, mas destes, dois mencionaram uma tendência para um peso da placenta superior em fetos masculinos. Discussão: Uma placenta mais pesada em fetos masculinos relaciona-se com um aumento da eficiência no crescimento do feto e, consequentemente, do seu peso ao nascimento. Todavia, este investimento no crescimento "quantitativo", em detrimento do crescimento "qualitativo", irá, em última instância, comprometer a gestação e a sua saúde na vida adulta. |
