| Title: | Diagnostic accuracy of cardiovascular magnetic resonance imaging in cardiac tumors: a systematic review with meta-analysis |

| Abstract: | Purpose: Cardiac tumours (CT) diagnosis's gold standard is histopathological examination. Cardiovascular magnetic resonance (CMR) is a valuable non-invasive radiation-free tool for identifying and characterising CT. Our aim is to understand how accurate CMR is when compared to the gold standard. Methods: A systematic search was performed in PubMed, Web of Science and Scopus databases up until December 2022 and the results were reviewed by two independent investigators. Studies reporting CMR accuracy were included in a meta-analysis and pooled measures were obtained. The risk of bias was assessed using the Quality Assessment Tools from the National Institutes of Health. Results: A total of 2321 results were obtained; ten studies were eligible, including one identified by citation search. Eight studies were included in the meta-analysis, which presented a pooled sensitivity of 93% and specificity of 94%, a diagnostic odds ratio of 185 and an AUC of 0.98 for CMR diagnosing benign vs malignant tumours. Additionally, 4 studies evaluated whether CMR diagnosis of CT matched specific histopathological subtypes, with 73.6% of correct diagnoses. Conclusion: To the best of our knowledge, this is the first published systematic review reporting on the accuracy of CMR in diagnosing CT. CMR showed high accuracy in the differentiation of benign and malignant tumours; though the capacity of distinguishing subtypes was not outstanding. However, substantial heterogeneity may affect our findings. |
| Description: | Objetivo: O método gold-standard para o diagnóstico dos tumores cardíacos (TC) é o exame histopatológico. A ressonância magnética cardiovascular (RMC) é uma valiosa ferramenta não invasiva e sem radiação na identificação e caraterização de TC. O nosso objetivo é compreender a acuidade diagnóstica da RMC quando comparado com o método gold-standard. Métodos: Uma pesquisa sistemática foi realizada no PubMed, Web of Science e Scopus até dezembro de 2022 e os resultados foram revistos por dois investigadores independentes. Os estudos que reportaram a acuidade da RMC foram incluídos na meta-análise e medidas sumárias foram obtidas. O risco de viés foi avaliado recorrendo à Quality Assessment Tools do National Institutes of Health. Resultados: Foram obtidos um total de 2321 resultados; dez estudos foram elegíveis, incluindo um identificado por pesquisa da bibliografia dos estudos incluídos. Oito estudos foram incluídos na meta-análise, que apresentou uma sensibilidade sumária de 93% e especificidade de 94%, um diagnostic odds ratio de 185 e uma AUC de 0,98 para o diagnóstico de tumores benignos versus malignos por RMC. Adicionalmente, 4 estudos avaliaram se o diagnóstico de TC por RMC coincidia com os subtipos histopatológicos, com 73,6% de diagnósticos corretos. Conclusão: Até onde sabemos, esta é a primeira revisão sistemática publicada aludindo à acuidade da RMC no diagnóstico de TC. A RMC mostrou elevada acuidade na diferenciação de tumores benignos e malignos, apesar de a capacidade de distinguir subtipos histológicos não se ter mostrado notável. Não obstante, heterogeneidade substancial pode afetar as nossas conclusões. |
