| Title: | THE IMPACT OF THE COVID-19 PANDEMIC IN THE CLINICAL ASSISTANCE TO BREAST CANCER PATIENTS. |

| Abstract: | ABSTRACT INTRODUCTION: The COVID-19 pandemic led to important health care access limitations. We aimed to disclose the impact of the pandemic on breast cancer patients in a specialized Breast Cancer Center (BCC). PATIENTS AND METHODS: A total of 501 breast cancer patients with a first appointment in the BCC from April 1st,2019 to March 31st, 2021 were divided into 4 consecutive periods of 6 months. Data from the homologous semesters was compared. Patients with an appointment in the BCC during the study period were eligible for the secondary aim of our study (BCC workload). RESULTS: After the pandemic declaration (period 3), we found a decrease in the referral by screening programs (p=0.002) and a reduction in the waiting time between the Primary Care (PC) referral and the first BCC appointment (p<0.001). There were higher rates of palpable axillary nodes (p=0.001), an increase in N stage 2 and 3 (p=0.050), and a trend for primary endocrine therapy as the first treatment (p=0.021) associated with higher rates of complete axillary node dissection (p=0.030). In period 4, there were more outward diagnoses (p=0.003), a higher rate of surgery as the first treatment (p=0.013) and fewer days between the PC referral and first BCC appointment (p=0.009). No differences were found in the number of patients received and treated in the BCC during the four periods. In period 4, the waiting time for surgery was lower (p=0.002). CONCLUSIONS: COVID-19 pandemic implied a more advanced nodal stage, which may be related to the delay in breast cancer screening. |
| Description: | INTRODUÇÃO: A pandemia por COVID-19 levou a importantes limitações no acesso aos cuidados de saúde. Com este estudo pretendemos avaliar o impacto da pandemia na assistência a pacientes com cancro de mama num Centro especializado em Cancro de Mama (BCC)- Centro da Mama do Centro Hospitalar do Hospital de São João. PACIENTES E MÉTODOS: Um total de 501 pacientes com cancro de mama, com a primeira consulta no BCC entre 1 de abril de 2019 a 31 de março de 2021 foram divididos em 4 períodos consecutivos de 6 meses. Os dados dos semestres homólogos foram comparados. Pacientes com consulta no BCC durante o período do estudo foram elegíveis para o objetivo secundário do nosso estudo (carga de trabalho do BCC). RESULTADOS: Após a declaração da pandemia (período 3), verificou-se diminuição do encaminhamento por programas de rastreio (p=0,002) e redução do tempo de espera entre a referência feita pelo Médico de Família (AP) e a primeira consulta no BCC (p<0,001). Houve maiores taxas de nódulos axilares palpáveis (p=0,001), um aumento nos estadios N 2 e 3 (p=0,050) e uma tendência para a terapia endócrina primária como primeiro tratamento (p=0,021), associada a maiores taxas de dissecção nodular axilar (p=0,030). No período 4, houve mais diagnósticos externos (p=0,003), uma maior taxa de cirurgia como primeiro tratamento (p=0,013) e menos dias entre a referência pelo Médico de Família e a primeira consulta de BCC (p=0,009). Não foram encontradas diferenças no número de pacientes recebidos e tratados no BCC durante os quatro períodos. No período 4, o tempo de espera para cirurgia foi menor (p=0,002). CONCLUSÕES: A pandemia de COVID-19 implicou um estadio nodal mais avançado, o que pode estar relacionado com atraso no rastreio do cancro da mama. |
