| dc.description | Objetivo: Avaliar a influência da inclinação lateral posterior da tíbia (LPTS) e do ângulo menisco-osso (MBA) no risco de rotura primária do ligamento cruzado anterior (LCA) na população adulta através do rácio LPTS-MBA. Métodos: Foi realizado um estudo caso-controlo retrospetivo com pacientes de um hospital terciário submetidos a cirurgia primária do LCA e que possuíam uma ressonância magnética (RM) pré-operatória. Esses pacientes foram emparelhados por idade e sexo numa proporção de 1:1 com pacientes que tinham uma ressonância magnética sem rotura do LCA. LPTS e MBA foram medidos na ressonância magnética. Os dados quantitativos estão apresentados como mediana ± intervalo interquartis. A identificação de fatores de risco independentes para rotura primária do LCA foi feita através de uma regressão logística multivariada. As curvas de características de operação do recetor detetaram quaisquer variáveis com forte capacidade discriminativa. Resultados: 95 pacientes com rotura primária do LCA confirmada na ressonância magnética foram emparelhados com 95 controlos (N = 190). A maioria era do sexo masculino, com uma idade mediana de 26 anos. No grupo de rotura do LCA, a mediana do rácio LPTS-MBA foi de 0,20 (intervalo interquartis (IQR), 0,11-0,37) versus 0,12 (IQR, 0,08-0,19) no grupo controlo (P <0,001). LPTS teve uma mediana de 4,20º no grupo de rotura do LCA (IQR, 2,05-7,35º) versus 2,90º nos controlos (IQR, 2,05-5,00º) (P=0,026) enquanto o MBA foi de 19º (IQR, 16-24º) versus 26º (IQR, 24-30º) (P<0,001), respetivamente. A regressão logística mostrou que LPTS (odds ratio (OR) 1,20, intervalo de confiança de 95% (IC) 1,03-1,42, P = 0,021) e MBA (OR 0,78, IC 95% 0,71-0,85, P <0,001) foram preditores independentes. A área sob a curva (AUC) do rácio LPTS-MBA foi de 0,69, superior à do LPTS sozinho (AUC=0,61), mas inferior à do MBA (AUC=0,82). Conclusões: O rácio LPTS-MBA está aumentado em pacientes com rotura primária do LCA. LPTS e MBA são preditores independentes de rotura primária do LCA. A relação LPTS-MBA é melhor do que LPTS sozinho para prever a rotura primária do LCA; no entanto, MBA foi a variável preditiva mais forte. Nível de evidência: III, estudo caso-controlo. | |
| dc.description.abstract | Purpose: To evaluate the influence of lateral posterior tibial slope (LPTS) and meniscal bone angle (MBA) on primary anterior cruciate ligament (ACL) tear risk in adult population through LTPS-MBA ratio. Methods: A retrospective case-control study was performed with patients from a tertiary hospital who underwent primary ACL surgery and had a pre-operatory magnetic resonance imaging (MRI). These subjects were matched by age and sex in a 1:1 ratio to patients who had a MRI without ACL tear. LPTS and MBA were measured on MRI scan. Quantitative data is presented as the median ± interquartile range. Identification of independent risk factors for primary ACL tear was performed using multivariable logistic regression. Receiver operating characteristics curves detected any variable with strong discriminative capacity. Results: 95 patients with primary ACL tear confirmed on MRI were matched with 95 controls (N=190). Most of them were males with a median age of 26 years. In ACL tear group, the median value of LPTS-MBA ratio was 0.20 (interquartile range (IQR), 0.11-0.37) versus 0.12 (IQR, 0.08-0.19) in control group (P<0.001). LPTS had a median value of 4.20º in ACL tear group (IQR, 2.05-7.35º) and 2.90º in controls (IQR, 2.05-5.00º) (P=0.026) while MBA was 19º (IQR, 16-24º) versus 26º (IQR, 24-30º) (P<0.001), respectively. Logistic regression showed that LPTS (odds ratio (OR) 1.20, 95% confidence interval (CI) 1.03-1.42, P=0.021) and MBA (OR 0.78, 95%CI 0.71-0.85, P<0.001) were independent predictors. Conclusions: LPTS-MBA ratio is increased in primary ACL tear patients. LPTS and MBA are independent predictors of primary ACL tear.
