Title: A cross-sectional study of medical students to assess their exposure to and knowledge of renal transplantation. Attitudes and perception about organ donation and transplantation. Future implications?

Abstract: Background: Organ shortage is a major limiting factor for organ transplantation. Public behaviour towards organ donation and transplantation (ODT) can be positively influenced by health care professionals. Knowledge and attitudes are better shaped at an earlier educational stage. Hence, this study surveys knowledge, exposure to and attitudes towards ODT whilst evaluating educational needs of medicine students. Methods: A cross-sectional survey was delivered online to 285 sixth-year medicine students from the Faculty of Medicine, University of Porto (FMUP) between November 2021 and February 2022. The survey consisted on 5 main sections: socio-demographic variables; knowledge about ODT; exposure to ODT; perceptions and attitudes towards ODT and students' curriculum perception. Results: Of the 180 responses (63%), 66% had previous education. A vast majority (93,4%) knew about different donor kinds and about the legal framework (97, 2%). Nonetheless, 43,3% had no knowledge about an organ shortage. Education and discussing ODT amongst their families correlated positively with knowledge about ODT. Despite a positive attitude (99,4%) not even half had discussed ODT with their relatives (45%). Finally, a majority consider their exposure inadequate (53,9%) and their knowledge subpar (59,5%), with 89,5% agreeing that ODT should be in the obligatory curriculum. Conclusions: This cross-sectional study highlighted a level of exposure to and knowledge of ODT that is subpar, notwithstanding a positive attitude accompanied by a willingness to integrate it in the obligatory curriculum. Future doctors ought to be given the knowledge to support ODT and have their attitudes positively shaped allowing them to advocate for it.
Description: Contexto: A escassez de órgãos é um dos principais factores limitantes para a transplantação de órgãos. O comportamento público para com a doação e transplantação de órgãos (DTO) pode ser influenciada positivamente por profissionais de saúde. Conhecimentos e atitudes são melhor moldados num estádio educacional mais precoce. Destarte, este estudo avalia o conhecimento, a exposição e as atitudes para com a DTO enquanto simultaneamente avalia as necessidades educacionais de estudantes de medicina Métodos: Um questionário transversal foi entregue online a 285 estudantes do sexto ano do Mestrado Integrado em Medicina na Faculdade de Medicina da Universidade do Porto entre Novembro de 2021 e Fevereiro de 2022. O questionário consistiu em 5 principais secções: variáveis sociodemográficas; conhecimento sobre DTO; exposição a DTO; percepção e atitudes perante DTO e percepção dos estudantes perante o currículo vigente. Resultados: Das 180 respostas (63%), 66% tiveram educação prévia. Uma grande maioria (93,4%) sabia sobre a diferença dos diferentes tipos de dador e acerca do quadro legal (97,2%). Não obstante, 43,3% não tinha conhecimento acerca da escassez de órgãos. Educação e discutir DTO dentro do seio familiar correlaciona-se positivamente com conhecimento sobre DTO. Apesar de uma atitude positiva (99,4%), nem metade teria discutido DTO com os seus familiares (45%). Finalmente, a maioria considera a sua exposição inadequada (53,9%) e o seu conhecimento abaixo do esperado (59,5%), com 89,5% concordando que DTO deveria estar integrado no currículo obrigatório. Conclusão: Este estudo observacional transversal realçou um nível de exposição e um conhecimento de DTO que está aquém do esperado, apesar de uma atitude positiva acompanhada por uma vontade de a integrar no currículo obrigatório. Futuros médicos devem ser capacitados com o conhecimento para suportar o processo de DTO e ter as suas atitudes moldadas positivamente permitindo-os advogar pela mesma.
