dc.descriptionIntrodução: Apesar do provável envolvimento dos cisteinil-leucotrienos (CysLT) na progressão da COVID-19, o seu papel mantém-se pouco explorado neste contexto. O objetivo deste estudo foi avaliar a concentração urinária de cisteinil-leucotrienos (U-CysLT) como marcadores de gravidade e potenciais alvos terapêuticos em doentes COVID-19 hospitalizados. O impacto da hipertensão e da oxigenação por membrana extracorporal veno-venosa (VV-ECMO) nos valores de U-CysLT também foram avaliados. Métodos: Amostras de sangue e urina ocasional foram colhidas em doentes COVID-19 "graves"(n=27), "críticos"(n=17) e "críticos em ECMO"(n=17) nos dias 1-2 (admissão), 2-4, 5-8 e posteriormente de forma semanal, e nos controlos (dadores de sangue, n=23) num único tempo. As concentrações de U-CysLT foram avaliadas por ELISA. Também foram avaliados scores e outcomes de prognóstico. Resultados: Apesar de não ter havido diferenças nas concentrações de U-CysLT à admissão entre grupos, estes valores estavam marcadamente elevados nos dias 3-4 e 5-8 em doentes críticos em ECMO em comparação com doentes graves (p<0.010). Entre os doentes críticos em ECMO, os hipertensos apresentaram valores significativamente mais altos de U-CysLT (p<0.05 vs normotensos nos dias 5-8). Durante a hospitalização, os valores de U-CysLT aumentaram significativamente nos grupos de doentes críticos, mas não no grupo de doentes graves. Durante a primeira semana de hospitalização, os valores de U-CysLT mostraram uma correlação positiva com o tempo de estadia na Unidade de Cuidados Intensivos (UCI) e tempo total de hospitalização nos doentes críticos, sendo também significativamente mais altos nos não-sobreviventes ao comparar todos os grupos de doentes. Conclusão: A COVID-19 crítica está associada a aumento de concentrações de U-CysLT durante a hospitalização, sobretudo em doentes hipertensos em ECMO. A associação entre U-CysLT com outcomes de maior gravidade realça a sua utilidade como biomarcadores para estratificação de risco e prognóstico, bem como alvos terapêuticos.
dc.description.abstractIntroduction: Despite the likely involvement of cysteinyl leukotrienes (CysLTs) in progressing COVID-19, they remain scarcely explored in this context. We aimed to evaluate urinary CysLT (U-CysLTs) as severity biomarkers and putative therapeutic targets in hospitalized COVID-19 patients. The impact of hypertension and veno-venous extracorporeal membrane oxygenation (VV-ECMO) on U-CysLTs was also assessed. Patients and methods: Blood and spot urine were collected in "severe" (n=27), "critically ill" (n=17) and "criticallly ill on VV-ECMO" (n=17) patients with COVID-19 at days 1-2 (admission), 3-4, 5-8 and weekly thereafter, and in controls (blood donors, n=23) at a single time point. U-CysLT were measured by ELISA. Prognostic scores and outcomes were also evaluated. Results: Although there were no differences on U-CysLTs between groups at admission, they were markedly increased at days 3-4 and 5-8 in "critically ill on VV-ECMO" patients compared to severe patients (p<0.010). Among "critically ill on VV-ECMO" patients, those who were hypertensive had higher U-CysLT values (p<0.05 vs normotensives, at days 5-8). Along hospitalization, U-CysLTs significantly increased in critical groups, but not in "severe" patients. U-CysLTs values during the first hospitalization week were positively correlated with ICU and total hospital length of stay among all critically ill patients, being also significantly higher in non-survivors than in survivors when comparing all patient groups. Conclusions: Critical COVID-19 is associated with increased U-CysLTs during hospitalization, especially in hypertensive patients on VV-ECMO. The positive association of U-CysLTs with severe outcomes underlines their usefulness as biomarkers for risk stratification and prognostication and also as therapeutic targets.
