| Title: | The role of Deep Transcranial Magnetic Stimulation in Depression- a systematic review |

| Abstract: | ABSTRACT Background: Depression is considered as a condition of extremely high prevalence which very often brings an enormous burden to patients. Current ways of treatment frequently fail to decrease depressive symptoms and are also associated with high levels of toxicity. There is a need for new alternative treatments aiming to improve the health of these treatment-resistant patients. DTMS (deep transcranial magnetic stimulation) arises itself as a new type of treatment with a potential which is yet to be fully explored. Methods: Two authors conducted a systematic literature search through three different databases. Articles that assessed the effects of dTMS in patients suffering from unipolar or bipolar depression through the Hamilton Depression Rating Scale were included. Main outcomes were HDRS score changes and rates of response, remission and adverse effects. Results: Almost every included study showed that dTMS had a significant effect in reducing depressive symptoms, with high rates of response and remission and a scarce number of adverse effects registered. Limitations: Most of our included studies were uncontrolled and had a low number of participants. In addition to that, follow-up periods were short considering depression is a chronic and relapsing disease. Conclusions: Our results show that dTMS has the potential to become an important tool for treatment-resistant depression in the future, with high rates of response and an overall high degree of safety. However, further studies with a higher number of involved patients are required in order to fully establish the clinical utility of this new form of treatment. |
| Description: | Introdução: A depressão é considerada como um distúrbio de prevalência extremamente elevada com um impacto bastante significativo na vida dos doentes. As formas atuais de tratamento frequentemente são ineficazes na diminuição dos sintomas depressivos e associam-se a altos níveis de toxicidade. Como tal, surge a necessidade de novos tratamentos alternativos com o objetivo de melhorar a saúde desses pacientes resistentes ao tratamento. A dTMS (Deep Transcranial Magnetic Stimulation) surge como um novo tipo de tratamento com um potencial ainda por explorar. Métodos: Dois autores realizaram uma revisão sistemática da literatura em três bases de dados diferentes. Foram incluídos artigos que avaliaram os efeitos da dTMS em pacientes com depressão unipolar ou bipolar por meio da Escala de Avaliação de Depressão de Hamilton. Os principais outcomes foram alterações no score HDRS e taxas de resposta, remissão e efeitos adversos. Resultados: Quase todos os estudos incluídos mostraram que a dTMS teve um efeito significativo na redução dos sintomas depressivos, com altas taxas de resposta e remissão e um número escasso de efeitos adversos registados. Limitações: A maioria dos nossos estudos incluídos não possuíam grupo de controlo e tinham um número baixo de participantes. Além disso, os períodos de seguimento foram curtos, tendo em conta que a depressão é uma doença crónica e recidivante. Conclusões: Os nossos resultados mostram que a dTMS tem potencial para se tornar uma ferramenta importante para a depressão resistente ao tratamento no futuro, com altas taxas de resposta e um alto grau de tolerãncia. No entanto, mais estudos com um número maior de pacientes envolvidos são necessários para estabelecer claramente a utilidade clínica desta nova forma de tratamento. |
