| Title: | Binge Eating Disorder and Appetite |

| Abstract: | Binge eating disorder (BED) is the most common of the 3 major eating disorders with an estimated lifetime prevalence of 0.85% and an estimated 12-month prevalence of 0.44%.(1) It's characterized by eating a large amount of food in a short period of time with perceived loss of control. Due to its increasing prevalence, it entails negative health outcomes and it's associated with an increasingly higher burden on healthcare systems. Although the etiology of BED has been put on psychological factors, emerging studies have been suggesting the multifactorial nature of BED and other eating disorders (EDs). In fact, appetite and food intake results from the complex interplay of homeostatic and hedonic systems, influenced by environmental, genetic, hormonal and psychological factors. Disordered eating behavior develops when there is an imbalance between these systems. In the present review, we focus on appetite regulation, particularly in the physiological and psychological bases which impairment may lead to development of binge eating behavior and therefore BED. Enlightening appetite dysregulation mechanisms and factors involved in development and maintenance of BED, is of great relevance in the discover of future target treatment approaches for this complex ED. |
| Description: | A perturbação do comportamento alimentar compulsiva ou Binge Eating Disorder (BED) é a mais comum das 3 principais perturbações do comportamento alimentar, com prevalência estimada ao longo da vida de 0,85% e prevalência estimada em 12 meses de 0,44%. Caracteriza-se pela ingestão de grande quantidade de alimentos num período de tempo reduzido associada a perceção de perda de controlo. Devido à sua prevalência crescente, associa-se a resultados negativos para a saúde, assim como a uma sobrecarga dos sistemas de saúde. Embora a etiologia da BED tenha vindo a ser atribuída a fatores psicológicos, estudos emergentes têm sugerido a natureza multifatorial desta e das restantes perturbações alimentares. De fato, o apetite e a ingestão alimentar resultam da complexa interação dos sistemas homeostático e hedónico, influenciados por fatores ambientais, genéticos, hormonais e psicológicos. Desta forma, a desregulação do comportamento alimentar desenvolve-se quando há um desequilíbrio entre estes sistemas, podendo evoluir posteriormente para perturbação do comportamento alimentar identificada. Neste trabalho, focamos na regulação do apetite, nomeadamente nas bases fisiológicas e psicológicas, cujo comprometimento pode levar ao desenvolvimento do comportamento de compulsão alimentar e, consequentemente a BED. O esclarecimento dos mecanismos de desregulação do apetite e dos fatores envolvidos no desenvolvimento e manutenção da BED é de grande relevância na descoberta de futuras abordagens de tratamento mais direcionadas e eficazes para esta complexa patologia. |
