| dc.description | As consequências neuropsiquiátricas a longo prazo da doença crítica são bem conhecidas. Assim, espera-se que os doentes críticos COVID-19 também possam apresentar vários sintomas psiquiátricos, como a depressão, com um efeito negativo inevitável na qualidade de vida relacionada com a saúde (HRQoL). Este estudo tem como objetivo identificar sintomas de depressão nestes sobreviventes e a sua associação com sintomas físicos pós-alta e HRQOL. Estes dados transversais fazem parte do projeto longitudinal MAPA com doentes críticos COVID-19 internados no Serviço de Medicina Intensiva (SMI) do CHUSJ (Outubro de 2020 a Agosto de 2021). Foram excluídos os doentes com estadia SMI≤24h, doença terminal, perda auditiva grave ou incapacidade de comunicação no momento de seguimento. Os participantes foram avaliados por telefone na consulta de seguimento (1-2 meses após a alta), com o Patient Health Questionnaire (PHQ-9) (depressão) e EuroQol 5-dimension 5-level EQ-5D-5L (HRQoL). No total, foram incluídos 131 doentes (idade média=62 anos) e 21% apresentaram sintomas depressivos. Os pacientes com sintomas depressivos mostraram mais problemas de autocuidado, dor/mal estar e ansiedade/depressão. Também relataram mais alterações do sono, dificuldades de memória, dispneia, disfonia, disfagia e problemas com o equilíbrio/ortostatismo. Estes resultados estão em linha com estudos anteriores em sobreviventes de COVID-19, com a depressão a ser associada com sintomas físicos e pior HRQoL. Assim, o rastreio precoce e o tratamento da depressão serão fundamentais para otimizar a recuperação dos pacientes e o HRQoL. | |
| dc.description.abstract | Long-term neuropsychiatric consequences of critical illness are well known. So, it is expected that critically ill COVID-19 patients might also present several psychiatric symptoms such as depression, with inevitable negative effect on health-related quality of life (HRQoL). This study aims to identify depression symptoms in these survivors and its association with post-discharge physical symptoms and HRQOL. This cross-sectional data is part of MAPA longitudinal project with critically ill COVID-19 patients admitted into Intensive Care Medicine Department( ICMD) of CHUSJ(October 2020 to August 2021). Patients with an ICMD stay≤24h, terminal illness, major auditory loss, or inability to communicate at follow-up time were excluded. Participants were evaluated by telephone at follow-up appointment (1-2 months post-discharge), with Patient Health Questionnaire (PHQ-9) (depression) and EuroQol 5-dimension 5-level EQ-5D-5L (HRQoL). Overall, 131 patients were included (median age=62y.o.) and 21% presented depressive symptoms. Patients with depressive symptoms showed more problems of self-care, pain/discomfort and anxiety/depression. Also reported more sleep alterations, memory difficulties,dyspnea,dysphonia, dysphagia, and trouble with balance/orthostatism. These results are in line with previous studies in COVID-19 survivors, with depression being associated with physical symptoms and worse HRQoL. Thus, early screening and treatment of depression will be fundamental to optimize the patients' recovery and HRQoL. | |
