Title: Recognizing the diversity of the Portuguese transgender population: A cross-sectional study

Abstract: The transgender population comprises anyone who wishes to be socially recognized as a distinct gender from their birth-assigned sex, with or without the desire of undergoing gender-affirming treatments. Its complexity is recognized, but remains an understudied topic, particularly in Portugal. The main goal of this study was to recognize and characterize the heterogeneity of this population, by exploring the possible gender identities, sexual orientations, and perspectives on the available treatments for gender dysphoria, including psychotherapy, hormone therapy, and sex-reassignment surgery. For this purpose, we applied an in-person, self-completion survey to 46 transgender individuals followed at the Psychiatry Department of the University Hospital Center of São João. The typical profile of our respondents was a young person, the majority being assigned female at birth, with upper secondary or higher education. Most of the patients were currently undergoing psychotherapy (89.1%) and hormone therapy (60.9%), but only a quarter had breast surgery in the past (26.1%). Satisfaction was lower towards breast surgery, but overall median satisfaction with all procedures was high. This study effectively demonstrates the diversity of the transgender population, pointing out differences in terms of sociodemographic data, gender expression, sexual attraction, treatment adherence and related satisfaction, while also gathering the necessary feedback to improve healthcare.
Description: A população transgénero inclui toda e qualquer pessoa que deseje ser socialmente reconhecida por um género distinto do sexo atribuído ao nascimento, independentemente das suas preferências relativamente a tratamentos de afirmação de género. A complexidade desta população é reconhecida, mas continua a ser um tema pouco estudado, particularmente em Portugal. O principal objetivo do presente estudo foi reconhecer e caracterizar a heterogeneidade da população transgénero, explorando as possíveis identidades de género, orientações sexuais e perspetivas sobre as intervenções atualmente disponíveis para a disforia de género, incluindo psicoterapia, hormonoterapia e cirurgia de redesignação sexual. Como tal, aplicámos presencialmente um questionário de autopreenchimento a 46 indivíduos transgénero acompanhados no Serviço de Psiquiatria do Centro Hospitalar Universitário de São João. O perfil típico dos nossos inquiridos foi um indivíduo jovem, maioritariamente do sexo feminino (atribuído ao nascimento), com ensino secundário ou superior. A generalidade dos pacientes encontrava-se a realizar psicoterapia (89,1%) e hormonoterapia (60,9%), mas apenas um quarto foi sujeito a cirurgia de mama no passado (26,1%). A satisfação foi menor em relação à cirurgia de mama, mas a satisfação mediana geral com todas as intervenções foi elevada. Este estudo comprova efetivamente a diversidade da população transgénero, apontando diferenças ao nível de dados sociodemográficos, expressão de género, atração sexual, adesão ao tratamento e respetiva satisfação, para além de reunir o feedback necessário para melhorar os cuidados de saúde a oferecer.
