| dc.description | Introdução: A disseção axilar dirigida combina a biópsia do gânglio sentinela com a remoção dos gânglios previamente marcados que estavam comprovadamente metastizados. A disseção axilar dirigida é um conceito promissor no estadiamento axilar de doentes com cancro da mama com metástases axilares e com resposta patológica completa ao tratamento neoadjuvante. Tivemos como objetivo avaliar a viabilidade da disseção axilar dirigida neste contexto. Métodos: Conduzimos um estudo observacional prospetivo em doentes cN1, confirmadas por biópsia, que se tornaram cN0 após tratamento neoadjuvante. A remoção do gânglio com clipe foi guiada por ecografia intraoperatória. A biópsia do gânglio sentinela usou verde de indocianina e azul patente. Se o gânglio com clipe ou os gânglios sentinela tivessem algum foco metastático, ou a disseção axilar dirigida fosse ineficaz, a doente era submetida a esvaziamento axilar. Resultados: Trinta e sete doentes foram incluídas. As taxas de identificação da disseção axilar dirigida e da biópsia do gânglio sentinela foram 97.3%. Todos os gânglios com clipe recuperados também foram gânglios sentinela. Ao nível individual, a taxa de identificação da biópsia do gânglio sentinela foi 89.2% com o verde de indocianina e 85.5% com o azul patente. A taxa de identificação do gânglio com clipe foi 81.1%, sendo mais alta quando este foi localizado com a ecografia intraoperatória (84.4% vs 60.0%). Uma resposta patológica completa axilar foi atingida em 54.1% das doentes e foi mais frequente em tumores HER2-positivos e triplos negativos (p=0.039). Dezanove doentes foram poupadas ao esvaziamento axilar. Conclusão: A disseção axilar dirigida com remoção guiada por ecografia intraoperatória do gânglio com clipe previamente marcado, e biópsia do gânglio sentinela com verde de indocianina e azul patente, é uma técnica viável e reprodutível para identificar doentes sem doença axilar residual após tratamento neoadjuvante e que podem ser poupadas ao esvaziamento axilar. | |
| dc.description.abstract | Purpose: Targeted axillary dissection (TAD) combines sentinel node biopsy (SNB) with the removal of the previously marked node proven to contain metastasis. TAD is a promising concept for axillary restaging in node-positive breast cancer patients with pathological complete response (pCR) to neoadjuvant therapy (NAT). We aimed to evaluate TAD feasibility in this context. Methods: A prospective observational study was conducted in biopsy-confirmed cN1 patients that became cN0 after NAT. The removal of the clipped node (CN) was guided by intraoperative ultrasound. SNB used indocyanine green and patent blue V dye. If the CN or sentinel lymph nodes (SLN) had any metastatic foci, or the TAD procedure was unsuccessful, the patient was submitted to axillary lymph node dissection (ALND). Results: Thirty-seven patients were included. TAD and SNB identification rates were 97.3%. Every retrieved CN was also a SLN. At the individual level, SNB identification rate was 89.2% with indocyanine green and 85.5% with patent blue V dye. The CN identification rate was 81.1%, being higher when the CN was localized on the intraoperative ultrasound (84.4% vs 60.0%). Nodal pCR was achieved by 54.1% of our patients and was more frequent in HER2 positive and triple negative tumors (p=0.039). Nineteen patients were spared from ALND. Conclusion: TAD with intraoperative ultrasound-guided excision of the previously marked CN and SNB with indocyanine green and patent blue V dye is a feasible and reproducible technique to identify patients with no axillary residual disease after NAT, that can be spared from ALND. | |
