Title: Behaviour of Crohn's Disease: the risk of postoperative recurrence

Abstract: Introduction: Crohn's Disease (CD) classification has been changing through the years. Nowadays, Montreal Classification classifies patients using three main parameters: age at diagnosis, disease location and disease behaviour. Medical treatment has been evolving a lot, with the introduction of new therapies that allow a better disease control. However, surgery is still necessary in a lot of patients and postoperative recurrence remains an important problem. Aim: The aim of this work is to verify the existence of a relationship between CD behaviour and postoperative recurrence. Materials and methods: A literature search was conducted in the MEDLINE database, using PubMed. A total of 59 papers were included in this work. Results and discussion: When comparing nonstricturing nonpenetrating, stricturing and penetrating behaviour, the last one was significantly associated with a higher rate of postoperative recurrence in most of the analysed papers. Nevertheless, further studies are necessary to evidence this relationship. Conclusion: The goal in CD treatment is to stratify patients taking into consideration their risk factors, in order to give them the best medical and surgical treatment and the best quality of life.
Description: Introdução: A classificação da Doença de Crohn (DC) tem evoluído ao longo dos anos. Atualmente, a Classificação de Montreal utiliza três parâmetros principais: idade de diagnóstico, localização e fenótipo de doença. O tratamento médico tem evoluído muito, com a introdução de novas terapêuticas que permitem um melhor controlo da doença. No entanto, a cirurgia continua a ser necessária em muitos doentes e a recorrência pós-operatória continua a ser um problema importante. Objetivo: O objetivo deste trabalho é verificar se existe uma relação entre o fenótipo da DC e a recorrência pós-operatória. Materiais e métodos: Foi conduzida uma pesquisa da literatura na base de dados da MEDLINE através da Pubmed. Um total de 59 artigos foram incluídos neste trabalho. Resultados e discussão: Quando comparados os fenótipos não estenosante não penetrante, estenosante e penetrante, este último foi significativamente associado com maior taxa de recorrência pós-operatória na maioria dos trabalhos analisados. Ainda assim, são necessários mais estudos para evidenciar esta relação. Conclusão: O objetivo no tratamento da DC é a estratificação dos doentes tendo em consideração os seus fatores de risco, com o objetivo de lhes dar o melhor tratamento médico e cirúrgico e a melhor qualidade de vida.
