Title: Long-Term Outcomes of Diabetic Macular Edema Treatment with Intravitreal Fluocinolone Acetonide: A Real-World Experience

Abstract: Purpose To report on the safety and efficacy of intravitreal fluocinolone acetonide (FAc) in eyes with chronic diabetic macular edema (DME). Methods This is a retrospective observational study of 16 eyes with chronic DME that had the intravitreal FAc implant for at least 3 years. The main outcome collected from clinical records were central macular thickness (CMT), visual acuity (VA), intra-ocular pressure (IOP) and retinal nerve fiber layer (RNFL) thickness. Results This study included 16 eyes from 12 patients with a mean age at the time of the implant of 70 years. Regarding efficacy, CMT had a statistically significant decrease (p=0,02) while VA remained stable overtime (p=0,13). Concerning safety, mean IOP did not significantly differ throughout the follow-up time (p=0,26), however, the number of eyes on topical antihypertensive drugs doubled. A slow decrease of RNFL thickness could be observed overtime, although not statistically or clinically relevant (p=0,17). Conclusion The implant has shown to be safe and anatomically efficient over three years for the treatment of DME and it stabilized VA. However, further studies are necessary to understand the predictors of response to FAc implantation and to elucidate the role of RNFL thickness in the prediction of glaucoma in DME eyes.
Description: Objetivo Avaliar a segurança e eficácia do implante intravítreo de acetonido de fluocinolona (AcF) em olhos com edema macular diabético (EMD) crónico. Métodos Trata-se de um estudo observacional retrospetivo de 16 olhos com EMD crónico e refratário e que tiveram o implante de AcF durante pelo menos 3 anos. Os resultados principais colhidos nos registos clínicos foram a espessura macular central (EMC), acuidade visual (AV), pressão intraocular (PIO) e espessura da camada de fibras nervosas da retina (CFNR). Resultados Este estudo incluiu 16 olhos de 12 doentes com uma idade média de 70 anos. Relativamente à eficácia, o EMC teve uma redução estatisticamente significativa (p=0,02) enquanto que a AV permaneceu estável ao longo do tempo (p=0,13). Relativamente à segurança, a PIO média não variou significativamente durante o seguimento (p=0,26), apesar do número de olhos a fazer medicação anti-hipertensiva tópica ter duplicado. Observou-se uma redução progressiva da espessura da CFNR, apesar de não ser clinicamente ou estatisticamente significativa (p=0,17). Conclusão Este implante mostrou que o implante de AcF é seguro e eficaz ao longo de três anos na abordagem anatómica do EMD e manteve a AV. Contudo, mais estudos são necessários para compreender os preditores de resposta ao AcF e para esclarecer o papel da espessura da CFNR no rastreio de glaucoma nos olhos com EMD.
