Transposition of the Great Arteries: Review of Cases from a Reference Center

Abstract

Introduction and Objectives: Dextro-transposition of the great arteries (d-TGA) is a common cyanotic congenital heart defect, with the aorta and the pulmonary trunk arising from the right and left ventricles, respectively. The arterial switch operation (ASO) allows for its anatomical correction with low morbi-mortality rates. Our main goal was to describe the characteristics and outcomes of patients with d-TGA treated in our center. Methods: We performed a single-center retrospective cohort study of newborns with d-TGA admitted to our hospital between January 2016 and December 2020. We reviewed patient's medical records and collected data on their characteristics, surgery, and pre- and postoperative periods. Results: We included 18 patients with d-TGA: nine patients had simple d-TGA, eight had d-TGA with ventricular septal defect (VSD) and one had complex d-TGA. ASO was performed in 17 patients and there was one preoperative death. The complications found in the early postoperative period were: 10 infection/sepsis, seven chylothorax, five neurological lesions, five dysrhythmia, four wound dehiscence, two acute kidney injury, two diaphragmic paralysis and one cardiac tamponade. There were three early reinterventions. In the late postoperative period, five (27.8%) patients had moderate-to-severe supravalvular pulmonary stenosis, with two needing reinterventions. There were four (22.0%) patients with aortic insufficiency and two (11.1%) with aortic root dilation. Conclusions: The management of d-TGA cases in our hospital was comparable to other reference centers and resulted in low mortality, without any postoperative deaths. In the early and late postoperative period, morbidity was low and there were few reinterventions.
